Confronto tra kit infusionale standard e kit extended in pazienti affetti da diabete mellito di tipo 1 in terapia con microinfusore Medtronic 780G: analisi del compenso glicemico.

BACKGROUND: La terapia con infusione continua di insulina sottocutanea rappresenta una delle opzioni più valide per il trattamento del diabete mellito di tipo 1 (DM1).
Nonostante gli innumerevoli progressi in ambito tecnologico, esistono ancora notevoli opportunità di sviluppo, soprattutto sulla durata dei set di infusione insulinica. Infatti, la maggior parte dei kit standard (IIS) in commercio è validata per un uso di tre giorni. Dal 2021 è stato introdotto un set di infusione esteso (EIS) con durata di sette giorni.
METODI: Tra febbraio e aprile 2023 sono stati valutati 52 pazienti affetti da DM1, età 18-70 anni, in terapia da almeno 6 mesi con ISS, seguiti dall’IRCCS San Raffaele di Milano. Sono stati valutati all’inizio dello studio (T0) diversi parametri, tra cui compenso glicemico, effetti collaterali, insulina residua al cambio set e impatto economico. In seguito, è stato effettuato il cambio di IIS con EIS, e dopo 30 giorni (T1), sono stati analizzati gli stessi parametri.
RISULTATI: I dati hanno evidenziato valori di emoglobina glicata e glicemia media pressochè invariati con l’utilizzo di EIS. Non sono state evidenziate reazioni avverse cutanee.
Inoltre, è stata riscontrata una riduzione del consumo di insulina legato alla minore presenza di farmaco residuo ad ogni cambio set.
A dispetto dei costi teorici annui sovrapponibili (calcolati su un corretto utilizzo dei set: 3 giorni per IIS e 7 giorni per EIS), dallo studio è emerso uno svantaggio in termini economici del nuovo set, poiché i pazienti hanno effettuato cambio di IIS ogni 3.92 giorni, mentre hanno usato EIS in media per 6.21 giorni.
CONCLUSIONI: L’EIS offre ottime prestazioni in termini di controllo glicemico, effetti indesiderati e risparmio annuo di insulina. La sua prescrizione sembra comportare un costo annuale maggiore, ma si ritiene che questo dato dipenda dalla necessità di selezionare adeguatamente i pazienti prima dell’avvio di EIS educandoli a sfruttarne meglio le caratteristiche BACKGROUND: Continuous subcutaneous insulin infusion therapy represents one of the most valid options for the treatment of type 1 diabetes mellitus (DM1).
Despite the countless advances in technology, there are still significant opportunities for development, especially on the duration of insulin infusion sets. In fact, most of the standard kits (IIS) on the market are validated for a three-day use. From 2021, an extended infusion set (EIS) with duration of seven days has been introduced.
METHODS: Between February and April 2023, 52 patients affected by DM1, aged 18-70 years, treated for at least 6 months with ISS, followed by the IRCCS San Raffaele of Milan were evaluated. Various parameters were evaluated at the beginning of the study (T0), including glycemic compensation, side effects, residual insulin at the set change and economic impact. Subsequently, the exchange of IIS with EIS was performed, and after 30 days (T1), the same parameters were analysed.
RESULTS: The data showed glycated hemoglobin and mean glycaemia values almost unchanged with the use of EIS. No adverse skin reactions were noted.
Furthermore, a reduction in insulin consumption was found linked to the lower presence of residual drug at each set change.
In spite of the overlapping theoretical annual costs (calculated on the correct use of the sets: 3 days for IIS and 7 days for EIS), the study revealed a disadvantage in economic terms of the new set, since the patients changed the IIS every 3.92 days, while they used EIS for an average of 6.21 days.
CONCLUSIONS: EIS offers excellent performance in terms of glycemic control, side effects and annual insulin savings. Its prescription seems to involve a higher annual cost, but it is believed that this data depends on the need to adequately select patients before starting EIS, educating them to better exploit its characteristics
