Pattern radiologici specifici nelle neoplasie polmonari con mutazione di KRAS

Nato da un’osservazione puramente clinica, questo studio ha come scopo quello di valutare nei pazienti con NSCLC e mutazione di KRAS la correlazione con lo sviluppo di alterazioni radiologiche a vetro smerigliato e se la comparsa di quest’ultime infici la sopravvivenza. Dopo aver selezionato 90 pazienti, sono state raccolte e valutate le scansioni TC basali antecedenti la terapia di I linea od il follow-up: 56 pazienti (62.2%) presentavano GGO e 33 pazienti (36.7%) non presentavano GGO, per un paziente (1.1%) il dato non era disponibile. Sono stati poi confrontati le OS dei pazienti senza GGO e di quelli con GGO nei vari sottogruppi, elaborando apposite curve di Kaplan-Meier. Nei pazienti trattati con terapia sistemica (n=70) è stata registrata una OS mediana di, rispettivamente, 18,57 vs 8,43 mesi (HR= 0.75; 95% CI, 0.43-1.32; p= 0.33). Nel sottogruppo trattato con immunoterapia (n=37) le OS mediane sono state, rispettivamente di 10,10 vs 7,13 mesi (HR= 0.76; 95% CI, 0.37-1.58; p= 0.47). Ma il dato interessante è situato nei pazienti che hanno ricevuto esclusivamente pembrolizumab (n=24) come trattamento di I linea: la PFS mediana nei pazienti senza GGO (n=10) rispetto a quelli con GGO (n=14) è stata di, rispettivamente, di 17,33 vs 0,70 mesi (HR= 0.52; 95% CI, 0.21-1.29; p= 0.15), mentre la OS mediana registrata è risultata, rispettivamente, di 23,17 vs 2,57 mesi (HR= 0.52; 95% CI, 0.21-1.28; p= 0.15).
In conclusione, è stata osservata una correlazione tra la mutazione di KRAS e lo sviluppo di GGO a livello radiologico. Sebbene non raggiungano la significatività statistica, i dati relativi alla sopravvivenza, non trascurabili dal punto di vista clinico, suggeriscono come lo sviluppo di GGO correli con una prognosi peggiore, specialmente nei pazienti trattati con immunoterapia, in particolare per quelli trattati solo con pembrolizumab. Si ritiene che studi successivi con un numero di pazienti maggiore possano confermare il dato statistico. Born from a purely clinical observation, this study aims to evaluate in patients with NSCLC and KRAS mutation the correlation with the development of ground-glass radiological alterations and if the appearance of the latter affect survival. After selecting 90 patients, baseline CT scans prior to first-line therapy or follow-up were collected and evaluated: 56 patients (62.2%) had GGO and 33 patients (36.7%) did not have GGO, for one patient (1.1%) the data was not available. The OS of patients without GGO and of those with GGO were then compared in the various subgroups, elaborating appropriate Kaplan-Meier curves. In patients receiving systemic therapy (n=70) there was a median OS of 18.57 vs 8.43 months, respectively (HR= 0.75; 95% CI, 0.43-1.32; p= 0.33). In the immunotherapy subgroup (n=37), median OS was 10.10 vs 7.13 months, respectively (HR= 0.76; 95% CI, 0.37-1.58; p= 0.47). But the interesting finding is located in patients who received exclusively pembrolizumab (n=24) as first-line treatment: the median PFS in patients without GGO (n=10) compared with those with GGO (n=14) was, respectively, 17.33 vs 0.70 months (HR= 0.52; 95% CI, 0.21-1.29; p= 0.15), while the median OS recorded was 23.17 vs 2.57 months, respectively (HR = 0.52; 95% CI, 0.21-1.28; p= 0.15).
In conclusion, a correlation was observed between the KRAS mutation and the development of GGO at the radiological level. Although they do not reach statistical significance, the survival data, not negligible from a clinical point of view, suggest that the development of GGO correlates with a worse prognosis, especially in patients treated with immunotherapy, especially those treated only with pembrolizumab. It is believed that subsequent studies with a larger number of patients can confirm the statistic data.
