L’impatto del trapianto di midollo autologo sulla microstruttura cerebrale nella sclerosi multipla aggressiva valutato tramite MRI quantitativa

SCOPO: Il trapianto autologo di cellule staminali ematopoietiche (AHSCT) è un trattamento efficace per la sclerosi multipla (SM) aggressiva. Tuttavia, comporta rischi significativi e può causare atrofia cerebrale nei primi mesi dopo la procedura, visualizzabile in risonanza magnetica (RM). Questo fenomeno potrebbe essere causato da neurotossicità o da pseudoatrofia. Questo studio mira a comprendere meglio l'impatto dell'AHSCT sulla microstruttura cerebrale analizzando i dati di RM di pazienti con SM trattati con AHSCT.
METODI: Sono stati analizzati i dati di RM di 17 pazienti con SM aggressiva sottoposti ad AHSCT tra il 2006 e il 2019 presso 2 Centri SM italiani. I pazienti hanno ricevuto un regime di condizionamento di intensità intermedia. Le sequenze di RM includevano T1 3D ad alta risoluzione, FLAIR 3D e DTI assiale. I volumi cerebrali (TB, GM, WM) sono stati calcolati utilizzando CAT-12 di SPM. I valori di anisotropia frazionale (FA) sono stati ottenuti dalla DTI.
RISULTATI: L'analisi volumetrica ha coinvolto 14 soggetti con 3 scansioni (follow-up medio: 626 giorni) e 8 soggetti con 6 scansioni (follow-up medio: 1794 giorni). Nei primi 6-12 mesi dopo l'AHSCT è stata osservata una significativa perdita di TBV, con una riduzione dei volumi di WM (p=0,003). Tuttavia, la FA, indicativa dell'integrità della WM, è rimasta stabile nello stesso periodo. Dopo la fase iniziale di perdita accelerata di TBV, l'atrofia cerebrale si è rallentata e ha raggiunto livelli simili all'invecchiamento normale.
CONCLUSIONE: L'AHSCT provoca una significativa perdita precoce di TBV principalmente a causa della riduzione del volume di WM. Non sono stati riscontrati cambiamenti microstrutturali nelle regioni WM tramite la DTI, suggerendo che la neurotossicità da chemioterapia non è il principale fattore di questa perdita di volume nei pazienti. Dopo l'iniziale atrofia cerebrale accelerata, la perdita di volume si stabilizza a livelli simili all'invecchiamento normale. AIM: Autologous hematopoietic stem cell transplant (AHSCT) has proven to be an effective treatment for aggressive multiple sclerosis (MS). It is associated with significant risks and an increased rate of brain atrophy in the first months after the procedure has been observed using MRI. Whether this phenomenon is caused by drug-induced neurotoxicity or pseudoatrophy is unclear. This study aimed to gain a better understanding of the impact of AHSCT on brain microstructure by analyzing MRI data in MS patients treated with AHSCT.
METHODS: MRI data were collected from 17 patients with aggressive MS who underwent AHSCT between 2006 and 2019 at 2 Italian MS Centers. Patients received an intermediate-intensity conditioning regimen (16 BEAM+ATG, 1 CY+ATG). MRI protocol included high-resolution fast-spoiled-gradient-echo 3D T1-weighted sequence, 3D FLAIR, and axial single-shot spin-echo echo-planar diffusion tensor imaging (DTI). Brain volumes (total brain -TB-, gray matter -GM-, and white matter -WM-) were derived using CAT-12, part of SPM. Fractional anisotropy (FA) values were obtained from DTI.
RESULTS: Volumetric analyses included 14 subjects with 3 MRI scans (mean follow-up: 626 days) and 8 subjects with 6 MRI scans (mean follow-up: 1794 days). A significant TBV loss was detected in the first 6-12 months after AHSCT, with decreased WM volumes (p=0.003). At the same timepoints, FA, which reflects WM integrity, remained unaltered. After the initial accelerated TBV loss, brain atrophy slowed down and reached levels comparable to normal aging.
CONCLUSION: Significant early TBV loss is common after AHSCT and is mainly driven by a reduction in WM volume. In these regions, diffusivity maps obtained from DTI showed no microstructural changes, suggesting that neurotoxicity from chemotherapy is not the main driver of BVL in these patients. After an initial accelerated brain atrophy, volume loss slows down to levels comparable to normal aging.
