VALUTAZIONE OUTCOME CLINICI DELLE MALATTIE INFIAMMATORIE CRONICHE INTESTINALI IN CENTRO DI TERZO LIVELLO

dc.description.abstract | Scopo dello studio: Valutare gli outcome clinici dei pazienti affetti da malattia infiammatoria cronica intestinale (IBD) seguiti dall’IBD-Unit dell’HSM di Genova. Valutazione di: 1.Rispetto degli standard internazionali di riferimento (ECCO-guidelines) di monitoraggio e terapia; 2.Outcome clinici dei pazienti. Metodi: analisi retrospettiva delle 782 visite gastroenterologiche dell’intero anno 2021 di 232 pazienti con IBD tramite stato clinico secondo score di Harvey-Bradshaw e partial-MAYO e marker di infiammazione(PCR e calprotectina fecale,FC). Risultati: nell’86,6% delle visite la malattia era in remissione: valori di FC>250µg/g e PCR≥5 mg/ml si sono dimostrati statisticamente significativi(P<0,01) nell’individuare attività di malattia in relazione alla clinica. Dei 122 pazienti UC, il 58,2% è in terapia topica e il 90,2% in sistemica con ASA: 92,6% non assume steroide sistemico; 9,9% assume immunomodulanti e 12,3% assume biologici (equiparabili). Dei 110 pz CD,59,1% non assumono ASA, mentre 54,5% è in terapia con biologico (prevalenza rispetto a immunomodulanti,9,1%:Δ46,3%). Nell’86,7% dei pz in terapia biologica per UC c’è associazione con ASA, a differenza del CD in solo 26,7%. In 10,7% delle visite è avvenuto switch terapeutico (sulla base delle condizioni clinico-strumentali) di cui ben 48,9% a biologico(rifiutato da 17,1% dei pz), maggiore di quella a steroidi (41,2%), impiegati esclusivamente per induzione della remissione. Solo in 7,8% delle visite si è prescritta endoscopia e solo 1,3%dei pz ha necessitato di chirurgia. Conclusioni: efficacia della IBD-Unit con buon outcome clinico globale grazie a stretto follow-up; significatività statistica di correlazione tra elevati valori di FC e PCR con attività di malattia e importanza del loro utilizzo nella pratica clinica; elevata tollerabilità di ASA; ruolo prevalente dei biologici rispetto a immunomodulanti soprattutto in pazienti affetti da CD; ancora parziale timore per i biologici da parte dei pazienti. | it_IT |
dc.description.abstract | Objective: to evalutate the clinical outcomes of patients with Inflammatory Bowel Disease (IBD) followed by the IBD-Unit of the HSM of Genoa. Evaluation of: 1. Compliance with international reference standards (ECCO-guidelines) for monitoring and therapy; 2. Clinical outcomes of patients. Methods: retrospective analysis of 782 gastroenterological visits in the year 2021 of 232 patients with IBD, evaluating clinical status (according to Harvey-Bradshaw and partial-MAYO scores) and inflammation markers (PCR and fecal calprotectin, FC). Results: in 86.6% of the visits the disease was in remission: FC values> 250µg/g and CRP values ≥5 mg/ml were statistically significant (P <0.01) in identifying disease activity in relation to clinic. Of the 122 UC patients, 58.2% are in topical therapy and 90.2% in systemic therapy with ASA: 92.6% are not assuming systemic steroids; 9.9% take immunomodulators and 12.3% take biological (comparable rates). Of the 110 CD patients, 59.1% do not take systemic ASA while 54.5% are in biologic therapy (clear prevalence compared to immunomodulators, 9.1%:Δ46,3%). In 86.7% of patients in biological therapy for UC there is an association with ASA, while for CD is in only 26.7%. In 10.7% of the visits there was a therapeutic switch (based on the clinical and instrumental conditions) of which 48.9% to biological (rejected by 17.1% of the patients), greater than that to steroids (41.2%), used exclusively for induction of remission. Endoscopy was necessary in only 7.8% of the visits and only 1.3% of the patients required surgery. Conclusions: efficacy of the IBD-Unit in the treatment of IBD with good overall clinical outcome thanks to close follow-up; statistical significance of correlation between high FC and CRP values with disease activity and importance of their use in clinical practice; high tolerability of ASA; prevalent role of biologics compared to immunomodulators especially in CD; patients still partially afraid of biological therapy. | en_UK |
