Alzheimer's disease (AD) is traditionally assessed through the cognitive domain, but motor deficits appear early along the disease continuum and may provide objective markers. Muscle synergies, which describe how motor commands are organized, have never been studied in AD. This thesis evaluates EMG-derived spatial muscle synergies, extracted during a virtual reality (VR) catching task, as an early biomarker of cognitive decline.
Sixty-one right-handed participants (22 healthy controls [HC], 21 with Mild Cognitive Impairment [MCI], 18 with AD) intercepted a virtual ball with the throwing avatar visible, occluded, or occluded with the ball travelling without gravity. Kinematic analyses included 60 participants; EMG analyses the 37 with a complete set of channels (15 HC, 14 MCI, 8 AD). Spatial synergies were extracted from twelve upper-limb muscles by non-negative matrix factorization (NMF), clustered in HC into five reference modules and matched to each patient synergy. Performance and two activation features, relative area and center of mass, were analyzed with linear mixed-effects models; individuals were classified by leave-one-subject-out linear discriminant analysis.
AD patients intercepted the ball as often as HC when the thrower was visible, but lost accuracy when it was occluded. Patients' synergies matched the HC modules in proportion and muscle composition. The few group differences concerned the relative area of the module shared by all participants, lower in the clinical groups, and its dependence on target side in two modules, larger in AD. Thrower visibility anticipated the activation of every module equally in all groups. Classification of individuals did not exceed chance.
The catching deficit in AD emerges when information on the throw is removed and is not accompanied by a detectable reorganization of the spatial modules.
