La colite ulcerosa: dalla diagnosi alla terapia.

Abstract

La presente tesi si propone di eseguire un’analisi il più possibile accurata della letteratura in materia di colite ulcerosa partendo dagli aspetti più generali, proseguendo con la diagnosi, i diversi meccanismi che compongono il quadro fisiopatologico della malattia, fino ad arrivare alla terapia ed in particolare alle categorie di farmaci più utilizzati. La ricerca degli articoli scientifici che sono stati utilizzati per la realizzazione della tesi è stata svolta tramite la consultazione di siti come PubMed per le riviste scientifiche e drugbank.ca per i farmaci. A tale ricerca è seguito lo studio del materiale ottenuto, con una prima distinzione delle parti utili ai fini della tesi. Occorre sottolineare che l’argomento ha vari aspetti ed è oggetto di continua ricerca da parte della comunità scientifica, anche per il fatto che la colite ulcerosa fa parte di un gruppo di malattie, le IBD (malattie infiammatorie intestinali), che mostrano una serie di similarità e differenze che ne consentono lo studio comparativo: ognuno di questi aspetti potrebbe essere oggetto di una singola tesi. Un aspetto importante è quello riguardante la ricerca di nuovi marcatori capaci di distinguere specificamente la colite ulcerosa da altre patologie come la sindrome del colon irritabile, o il morbo di Crohn: lo studio della efficacia diagnostica di tali marcatori riporta sempre il confronto con una patologia infiammatoria che condivida similarità con quella in studio, senza trascurare il fatto che la terapia stessa spesso utilizza gli stessi farmaci o farmaci con un meccanismo d’azione simile.

This thesis aims to perform as accurate an analysis as possible of the literature on ulcerative colitis starting from the most general aspects, continuing with the diagnosis, the different mechanisms that make up the pathophysiological picture of the disease, up to the therapy and in particular to the categories of most used drugs. The search for scientific articles that were used to create the thesis was carried out by consulting sites such as PubMed for scientific journals and drugbank.ca for drugs. This research was followed by the study of the material obtained, with an initial distinction of the parts useful for the purposes of the thesis. It should be underlined that the topic has various aspects and is the subject of continuous research by the scientific community, also due to the fact that ulcerative colitis is part of a group of diseases, IBD (inflammatory bowel diseases), which show a series of similarities and differences that do not allow comparative study: each of these aspects could be the subject of a single thesis. An important aspect concerns the search for new markers capable of specifically distinguishing ulcerative colitis from other pathologies such as irritable bowel syndrome or Crohn's disease: the study of the diagnostic efficacy of such markers always involves a comparison with a pathology inflammatory that shares similarities with the one under study, without neglecting the fact that the therapy itself often uses the same drugs or drugs with a similar mechanism of action. Particular attention was paid to reporting the drugs used most in therapy and especially those currently used. Another interesting aspect that seemed right to report is the psychological and social impact of this pathology on patients, an aspect that is sometimes overlooked due to the need for care, but which is important for a good quality of life.
