dc.description.abstract | Background e obiettivi: Non sono ancora stati riportati dati su sicurezza ed efficacia del passaggio a OCR in pz con evidenza di attività/progressione di malattia dopo due cicli di alemtuzumab: il nostro studio ha cercato di fornire questi dati.
Metodi: I risultati descrittivi sono stati riportati come media con deviazione standard o mediana con intervallo interquartile. La probabilità di sopravvivenza libera da peggioramento della disabilità, sopravvivenza libera da ricaduta, sopravvivenza libera da attività di risonanza magnetica e stato NEDA-3 è stata calcolata con lo stimatore di Kaplan-Meier. Le analisi univariate e multivariate per valutare l'associazione delle caratteristiche demografiche e correlate alla malattia con gli endpoint di sopravvivenza sono state eseguite utilizzando modelli di regressione di Cox proportional hazards.
Risultati: È stata osservata una significativa diminuzione della conta totale dei linfociti. Questo declino è attribuito principalmente ai linfociti B CD18+.Non sono state osservate differenze significative nelle IgG e nelle IgA, bensì una significativa diminuzione dei livelli di IgM. Il 92,1% dei pazienti con SMRR era libero da recidive a 2 anni di FU. Nessun paziente con SM progressiva ha mostrato recidive durante il trattamento OCR. Il 90,8% dei pz con SMRR era privo di evidenza di attività della malattia alla RM e nessuno dei pazienti con SM progressiva mostrava evidenza di attività alla RM durante il trattamento con OCR. | it_IT |
dc.description.abstract | Background and aims: No safety and efficacy data about the switch to ocrelizumab in patients with evidence of disease activity/progression after two alemtuzumab courses have been reported yet. The aim of our study was to provide effectiveness and safety data of ocrelizumab treatment in patients with relapsing-remitting and progressive multiple sclerosis , who started OCR treatment due to persistent of MS activity after the second ALM course.
Methods: Descriptive results were reported as mean with standard deviation or median with interquartile range. The probability of disability worsening-free survival, relapsefree survival, MRI activity-free survival, and NEDA-3 status was calculated with the Kaplan–Meier estimator. Univariate and multivariate analyses assessing the association of demographic- and disease-related characteristics with survival endpoints were performed using Cox proportional hazards regression analysis models.
Results: A significant decrease in total lymphocytes count was observed. This decline is mainly attributed to lymphocytes B CD18+. No significant differences in IgG and IgA were observed, but a significant decrease in IgM levels was observed. A total of 92.1% patients with RRMS were free of relapses at 2-year FU. No PMS patients showed relapses during OCR treatment. At 2-year FU, 90.8% RRMS patients were free of MRI evidence of disease activity and none of the PMS patients showed evidence of MRI activity during OCR treatment.
Conclusions: The switch to OCR after 2 ALM courses is safe, no differences in terms of infections and others AEs with respect to clinical trials and real-lie available findings were observed. Our results suggest that ocrelizumab is a safe and effective treatment in patients with RRMS and progressive MS with a manageable safety profile. | en_UK |
