La metodica dello split crest con l'utilizzo degli espansori tronco conici

Abstract

La tesi con titolo “La metodica dello Split Crest con l’utilizzo degli espansori troncoconici” è una tesi incentrata sull’utilizzo degli espansori ideato dal dr. Sentineri con lo scopo di avere un’espansione delle creste atrofiche. Abbiamo selezionato vari casi in base all’anatomia delle creste edentule. I pazienti selezionati sono stati scelti secondo la “Classificazione dei biotipi ossei” e in particolare rientravano nel biotipo I e biotipo II. I 3 pazienti che abbiamo preso in esame erano pazienti senza problemi di salute e che non erano in terapia con farmaci che potessero interferire con la guarigione o nell’intervento. La tecnica chirurgica consente un allargamento della cresta con innesto di bioosso (osso eterologo) e copertura con membrana in CGF. Gli impianti sono stati inseriti contestualmente all’espansione di cresta. In tutti i casi clinici tale tecnica è stata predicibile con ottimi risultati e presenta notevoli vantaggi rispetto la tecnica tradizionale con martello e scalpello. Questi nuovi espansori hanno caratteristiche particolari che avendo la parte liscia coronale evitano la necrosi della corticale dovuto alle spire degli espansori precedenti , inoltre questa parte liscia favorisce il dislocamento delle corticali con un movimento di lateralità. Inoltra la membrana in CGF , confermato da questi casi clinici, consente una guarigione più rapida , una stabilizzazione dei tessuti e il rilascio di citochine nel tempo che favorisce un rimodellamento del tessuto connettivale ed osseo. La protesizzazione di questi casi , essendo casi recenti, deve ancora avvenire e avverrà dopo 5 mesi dall’intervento con la scopertura degli impianti.

The thesis entitled "The Split Crest method with the use of truncated cone expanders" is a thesis focused on the use of expanders devised by dr. Sentineri with the aim of having an expansion of the atrophic crests. We selected various cases based on the anatomy of the edentulous ridges. The selected patients were chosen according to the "Classification of bone biotypes" and in particular belonged to biotype I and biotype II. The 3 patients we examined were patients without health problems and who were not receiving medications that could interfere with healing or surgery. The surgical technique allows for an enlargement of the crest with a bio-bone graft (heterologous bone) and coverage with a CGF membrane. The implants were placed at the same time as the ridge expansion. In all clinical cases this technique has been predictable with excellent results and has significant advantages over the traditional technique with hammer and chisel. These new expanders have particular characteristics which, having the smooth coronal part, avoid the necrosis of the cortex due to the coils of the previous expanders, furthermore this smooth part favors the displacement of the cortices with a lateral movement. Relay membrane in CGF, confirmed by these clinical cases, allows for faster healing, tissue stabilization and release of cytokines over time which promotes a remodeling of connective tissue and bone. The prosthesis of these cases, being recent cases, has yet to take place and will take place 5 months after the intervention with the uncovering of the implants.
