La malattia di Paget: approccio dermatologico e senologico

Abstract

La malattia di Paget del capezzolo è un raro tumore cutaneo localizzato a livello del capezzolo, ma che può estendersi all’areola e alla cute circostante, associato ad un sottostante carcinoma. Studi hanno dimostrato la presenza di malattia di Paget nell’ 1-3% dei casi di carcinoma mammario. Si presenta quasi esclusivamente nelle donne e spesso è riconosciuto tardivamente. Per tale motivo, instaurare un iter diagnostico rapido e approfondito è di estrema importanza, ma spesso il dermatologo, che potrebbe aiutare nella diagnosi precoce, non viene consultato. Gli obiettivi principali di questo studio sono stati: Raccogliere i casi di malattia di Paget operati presso la Brest Unit dell’ Ospedale Policlinico San Martino-IRCCS, e confrontarli con i dati presenti in letteratura. Indagare se le pazienti al momento della comparsa della malattia cutanea, si fossero recate primariamente dal dermatologo o dal senologo. Per tale motivo sono stati raccolti tutti i casi di malattia di Paget diagnosticati istologicamente tra il 2012 ed il 2020. Successivamente, tutte le pazienti sono state contattate telefonicamente ed è stato chiesto loro, qual è stata la figura professionale alla quale si sono rivolte alla comparsa della malattia cutanea. Negli 8 anni di osservazione, i casi di malattia di Paget sono stati 45, le pazienti erano tutte di sesso femminile con età media al momento della diagnosi di 60±15 anni. Al momento dell’ intervista telefonica, la maggior parte delle pazienti (60,5%) ha dichiarato di essersi rivolta al senologo prima del dermatologo, sebbene la patologia interessi la cute. Poiché questo studio indaga solamente i casi di Paget confermati dall’istologia, non è dato sapere quante biopsie, fatte per sospetto di Paget, siano risultate negative. Si può però speculare che con una più stretta collaborazione tra dermatologi e senologi si potrebbero evitare esami invasivi inutili per alcune pazienti e velocizzare l'iter diagnostico terapeutico per altre.

The mammary Paget disease is a rare skin cancer, localized at the level of the nipple, but can also extend to the areola and the surrounding skin, associated to an underlying breast cancer. Studies have proven the presence of Paget disease in 1-3% of breast cancer cases. It almost exclusively occurs in the female patients and it’s usually recognized too late. For this reason, it’s extremely important to establish a rapid and in-depth diagnostic process, yet often the dermatologist, who can help make an early diagnosis, it’s not even consulted. The main objectives of this study were: Collecting the cases of Paget disease surgically operated in the Breast Unit of the Policlinico San Martino-IRCCS hospital, and compare them to the literature’s data. Investigate if the patients, at the onset of the skin disease, went first to the dermatologist or to the senologist. For that reason, all the histologically diagnosed cases of Paget disease from the 2012 to the 2020 have been gathered. Afterwards, all the patients were called by telephone and they were asked which professional figure they turned to when the skin disease appeared. In the 8 years of observation, the cases of Paget disease were 45, all patients were female with mean age at the time of diagnosis of 60±15 years. At the time of the telephone interview, most of the patients (60,5%) stated that they went to the senologist before the dermatologist, although the pathology affected the skin. Since this study investigate only the histologically confirmed cases of Paget disease, it’s not known how many biopsies, done on Paget's suspicion, were negative. However, it can be speculated that with closer collaboration between dermatologists and senologists, useless invasive examinations could be avoided for some patients and the diagnostic-therapeutic procedure could be speeded up for others.
