Stato di Her-2 nel carcinoma colo-rettale: risultati di uno studio monocentrico.

Abstract

Il carcinoma colo-rettale (CCR) è il terzo tumore più frequente e una delle principali cause di decesso per cancro in tutto il mondo. La proteina HER2 appartiene alla famiglia EGFR: una sua iper-espressione determina un aumento della sopravvivenza, della proliferazione e della progressione del tumore. Trastuzumab è un anticorpo monoclonale che agisce su HER2; si è rivelato importante nel trattamento del carcinoma mammario e, successivamente, nel trattamento del carcinoma gastrico, entrambi con iperespressione di HER2 (HER2 3+ e HER2 2+ amplificato). Recenti ricerche hanno evidenziato come lo status HER2 low (HER2 2+ non amplificato e HER 1+) possa beneficiare di una nuova strategia terapeutica (Trastuzumab+deruxtecan) sia nel carcinoma mammario sia in quello gastrico. Per questo motivi è interessante capire se queste strategie terapeutiche possano essere applicate anche nel CCR. Questo studio mira a determinare l’incidenza dell’iper-espressione di HER2 nel CCR, il suo impatto prognostico e la sua correlazione con alterazioni istologiche. Verrà valutata anche l’incidenza di HER2 low. Lo studio è stato effettuato su una ampia casistica monocentrica. Sono stati arruolati 1381 casi di CCR da luglio 2016 a giugno 2022, sottoposti ad intervento chirurgico di resezione presso l’Ospedale Policlinico IRCCS S. Martino di Genova. Lo stato di HER2 è stato valutato attraverso immunoistochimica e, nei casi debolmente positivi (HER2 +), ibridazione in situ (FISH). Nel complesso l’iper-espressione di HER2 è stata riscontrata in 62 pazienti (4.5%) della casistica. Non sono state evidenziate correlazioni significative tra espressione di HER2 e le altre variabili cliniche e patologiche, fatta eccezione per il sesso. Per quanto concerne il rapporto sopravvivenza ed iperespressione di HER2, nel nostro studio, non è stata rilevata nessuna correlazione. Lo score HER2 low invece è ancora poco studiato nel CCR: nel nostro studio è stato riscontrato in 518/1381 pazienti (37,5%).

Colorectal cancer (CRC) is a leading cause of cancer death worldwide. The HER2 protein belongs to the family of EGFR; its overexpression determines an increase in survival, proliferation and progression of the tumor. Trastuzumab is a monoclonal antibody that acts on HER2; it has proved to be extremely important in the treatment of breast cancer and, subsequently, in the treatment of gastric cancer, both with over-expression of HER2 (HER2 3+ and amplified HER2 2+). Recent research has shown that HER2 low status (unamplified HER2 2+ and HER 1+) can benefit from a new therapeutic strategy (Trastuzumab+deruxtecan) in both breast and gastric cancer. For this reason, it is interesting to understand whether these therapeutic strategies can also be applied in the CRC. This study is aimed to determine the incidence of HER2 overexpression in colorectal cancer, its prognostic impact and its correlation with other histological parameters. The incidence of HER2 low has also been evaluated. The study was carried out on a large monocenter series with evaluation of the histological variables by two expert pathologists. 1381 cases of CRC, surgically resected at the IRCCS Ospedale Policlinico S. Martino from July 2016 to June 2022, have been enrolled. HER2 status was assessed by immunohistochemistry and, in weak positive cases (HER2+), by in situ hybridization (FISH). HER2 overexpression was observed in 62 patients (4.5%) of the court. No significant correlations were seen between histological variables and HER2 status, except for sex. As far as the relationship between survival and HER2 overexpression is concerned, no correlation was found in our study. The HER2 low score, on the other hand, is still little studied in the CRC: in our study, the frequency was found in 518/1381 patients (37.5%).
