STESURA DI QUESTIONARI PER LA VALUTAZIONE DELL'APPROCCIO ORTODONTICO AL PAZIENTE CON IL DISTURBO ALLO SPETTRO AUTISTICO

Abstract

Questo studio vuole presentare alla comunità scientifica tre questionari sperimentali stilati per poter valutare l’efficacia dell’approccio ortodontico, intrapreso come primo atto terapeutico, nella gestione odontoiatrica di bambini con diagnosi di disturbo dello spettro autistico (ASD). L’ipotesi di poter guadagnare una maggior collaborazione da parte dei bambini con diagnosi di ASD, iniziando l’iter terapeutico dal trattamento ortodontico, nasce dall’osservazione del comportamento di questi pazienti e delle loro famiglie/ care givers quando si presentano all’Ambulatorio di Odontostomatologia e Ortodonzia pediatrica dell’IRCCS Gaslini di Genova. Com’è noto, l’odontoiatria pediatrica deve affrontare il difficile problema della scarsa collaborazione offerta dai piccoli pazienti a causa della loro età o problematiche cognitivo-comportamentali; ulteriori difficoltà insorgono durante gli atti terapeutici, in alcuni casi infatti è necessario l’uso di strumenti rotanti taglienti e rumorosi che possono generare paure e mettere a rischio l’integrità del paziente, nel caso in cui esso abbia un atteggiamento oppositivo al trattamento. A tal fine si è ipotizzata una nuova tecnica per migliorare il rapporto odontoiatra pediatrico-bambino ASD, iniziare il percorso terapeutico di questi pazienti partendo dal trattamento ortodontico nei casi in cui risulti necessario ed è possibile differire le altre terapie odontoiatriche. Questo approccio potrebbe essere utile per guadagnare la fiducia e la collaborazione del bambino affetto da ASD grazie alle caratteristiche delle visite ortodontiche: prevedono appuntamenti periodici, le manovre terapeutiche sono semplici, indolori e ripetitive, tutto ciò facilita una graduale familiarizzazione tra il bambino, l’operatore e l’ambiente odontoiatrico.

This study aims to present to the scientific community three experimental questionnaires drawn up in order to evaluate the effectiveness of the orthodontic approach, undertaken as a first therapeutic act, in the dental management of children diagnosed with autism spectrum disorder (ASD). The hypothesis of being able to gain greater collaboration from children diagnosed with ASD, starting the therapeutic process from orthodontic treatment, arises from the observation of the behavior of these patients and their families/caregivers when they present themselves at the Dentistry and Pediatric Orthodontics of the IRCCS Gaslini of Genoa. As is known, pediatric dentistry has to face the difficult problem of the poor collaboration offered by young patients due to their age or cognitive-behavioral problems; further difficulties arise during the therapeutic acts, in some cases it is in fact necessary to use sharp and noisy rotary instruments which can generate fear and put the patient's integrity at risk, in the event that he has an oppositional attitude to the treatment. To this end, a new technique has been hypothesized to improve the relationship between pediatric dentists and ASD children, to start the therapeutic course of these patients starting from orthodontic treatment in cases where it is necessary and it is possible to defer other dental therapies. This approach could be useful for gaining the trust and collaboration of the child affected by ASD thanks to the characteristics of the orthodontic visits: they require periodic appointments, the therapeutic maneuvers are simple, painless and repetitive, all this facilitates a gradual familiarization between the child, the operator and the dental environment.
