Gli usi compassionevoli del farmaco Luspatercept in Liguria prima della sua autorizzazione e conferma della sua efficacia. Luspatercept: un farmaco innovativo; relazione sui suoi usi più precoci in Liguria.

Abstract

Sono stati valutati i dati relativi agli usi compassionevoli del farmaco luspatercept richiesti in Liguria prima della sua autorizzazione. Luspatercept agisce legando e sequestrando vari ligandi effettori di Smad 2/3 inducendo il blocco del suo segnale di inibizione sull’eritropoiesi; ciò aumenta la maturazione dei precursori eritroidi. Nel periodo ottobre 2020 - giugno 2021 le richieste di uso compassionevole di luspatercept giunte al Comitato Etico della Liguria sono state 12; esse provenivano dalle Ematologie degli Ospedali S. Martino e Galliera e riguardavano pazienti affetti da grave anemia trasfusione dipendente dovuta a sindromi mielodisplastiche in 5 casi o -talassemia nei restanti 7 casi. Benché non si riscontrassero globalmente differenze importanti di sesso nei due gruppi, il numero di soggetti di sesso maschile era preponderante fra i 7 pazienti affetti da beta-talassemia. Le età medie dei due gruppi di pazienti erano molto differenti; infatti, i pazienti con sindromi mielodisplastiche avevano un’età media di 79,4±6,50 anni, mentre nei pazienti con talassemia l’età media era di 43,57±14,88 anni. L’analisi dei risultati della terapia con luspatercept si è concentrata sui 7 pazienti affetti da talassemia, evidenziando in particolare la durata del trattamento, la comparsa di effetti avversi e in genere l’efficacia del farmaco. In 3 pazienti il farmaco ha circa dimezzato la necessità di trasfusioni, mentre un paziente è risultato refrattario al trattamento. Due pazienti hanno interrotto il trattamento per motivi personali, mentre un altro paziente ha interrotto luspatercept per tossicità; ha infatti manifestato dolore articolare intenso, classificato di gravità 3 ed ha rifiutato di continuare l’assunzione del farmaco. Possiamo concludere che luspatercept ha confermato, in questo uso compassionevole, quanto già evidenziato negli studi registrativi, cioè una generalmente buona efficacia, con effetti indesiderati relativamente contenuti.

The data relating to the compassionate uses of the drug luspatercept required in Liguria before its authorization were evaluated. Luspatercept acts by binding and sequestering various effector ligands of Smad 2/3 inducing the blocking of its inhibition signal on erythropoiesis; this increases the maturation of erythroid precursors. In the period October 2020 - June 2021 the applications for compassionate use of luspatercept reached the Ethical Committee of Liguria were 12; They came from the Hematology of Hospitals S. Martino and Galliera and concerned patients suffering from severe anaemia transfusion dependent due to myelodysplastic syndromes in 5 cases or -thalassemia in the remaining 7 cases. Although there were no major gender differences in the two groups overall, the number of male subjects was predominant among the 7 beta-thalassemia patients. The average ages of the two groups of patients were very different, in fact patients with myelodysplastic syndromes had an average age of 79.4±6.50 years, while in patients with thalassemia the average age was 43.57±14.88 years. The analysis of the results of luspatercept therapy focused on 7 patients with thalassemia, highlighting in particular the duration of treatment, the appearance of adverse effects and generally the effectiveness of the drug. In 3 patients the drug approximately halved the need for transfusions, while one patient was refractory to treatment. Two patients stopped treatment for personal reasons, while another patient stopped luspatercept due to toxicity; in fact, he showed intense joint pain, classified as severity 3 and refused to continue taking the drug. We can conclude that luspatercept has confirmed, in this compassionate use, what already evidenced in the recording studies, that is, a generally good efficacy, with relatively low side effects.
