Analisi 3D degli effetti dell'espansore palatale custom: confronto fra l'ancoraggio sui molari decidui rispetto ai molari permanenti

Abstract

ANALISI 3D DEGLI EFFETTI DELL’ESPANSORE PALATALE CUSTOM: CONFRONTO FRA L’ANCORAGGIO SUI MOLARI DECIDUI RISPETTO AI MOLARI PERMANENTI Candidato: Manuel Silvani Relatore: Prof. Armando Silvestrini Biavati Correlatore: Prof. Marco Migliorati Introduzione e scopo della tesi Lo scopo di questo studio è studiare il livello di espansione e il cambiamento dell’inclinazione degli elementi dentari usando due tipi di espansori custom in pazienti in dentizione mista. Metodi Il primo gruppo con 20 pazienti ha un espansore mascellare con bande custom sui primi molari con appoggi sui premolari fino al canino e viene attivato una volta al giorno. Il secondo gruppo di 21 pazienti presenta un dispositivo di espansione mascellare con bande custom sui secondi molari decidui con appoggi sul premolare e canino e viene attivato due volte al giorno.In entrambi i gruppi i pazienti inclusi nello studio presentavano dentizione mista. Sono state eseguite scansioni prima di inserire l'espansore (T0) e dopo averlo rimosso (T1). Per ogni gruppo sono state misurate le distanze tra canini premolari e molari prima e dopo espansione. Inoltre abbiamo valutato il tip e il torque di ogni dente prima e dopo l’espansione e con un test t abbiamo valutato le differenze fra i due gruppi. Risultati abbiamo valutato l’aumento dell’espansione mascellare e non ci sono state differenze statisticamente significative. Neanche nel tip dei denti non ci sono state differenze statisticamente significative fra i due gruppi. Invece valutando la differenza di torque fra i due gruppi di 1.5 2.4 2.5 è stato trovato un p-value < 0,05 Conclusioni l'espansione palatale che si ottiene con una o due attivazioni è la stessa. Nel tip non ci sono state differenze statisticamente significative tra i due gruppi ma si è visto che l'espansore causa una variazione del torque perciò quando i denti decidui sono stabili si consiglia di usare un espansore digitale ancorato su di essi per ridurre l'inclinazione dei denti permanenti

ANALYSIS 3D OF THE EFFECT OF THE PALATAL EXPANDER CUSTOM MADE: COMPARISON BETWEEN ANCHORAGE ON PERMANENT MOLAR VERSUS DECIDUOUS MOLAR Candidate: Manuel Silvani Director: Prof. Armando Silvestrini Biavati Co-director: Prof. Marco Migliorati Objective The aim of this work is to evaluate the expansion and the inclination of the anchoring teeth of two different expander custom made in patients with mixed dentition. Methods the first group has 20 patient whit a palatal expander whit bands custom made on the first molar whit one activation daily . The second group has 21 patient whit a palatal expander whit bands custom made on the second molar deciduous whit two activation daily; in both group the patients are in mixed dentition. I made a digital impression before inserting the expander (T0) and another one after removing the palatal expander (T1). For each group I measured the distance between the canine, the premolar and the molar before and after the expansion. Furthermore for each couple of tooth I measured the distance between the palatal surface, the distance between the center of the teeth, and the distance between the mesial-vestibular cusp. Moreover I use the program OnyxCeph to complete the crown whit their roots in order to evaluate tip and torque of each teeth before and after the expansion. Result There are no statistically significant difference in the palatal expansion and neither in tipping but there are difference in the torque of 1.5 2.4 2.5 where the p-value is minor of 0,05 Conclusion The palatal expansion whit one activation or two is the same. The position of the band does not changes the tip but it influence the torque. That is why at the end of this study we suggest to use a digital expander anchored on deciduous tooth in order to reduce the side effect of the expansion
