dc.description.abstractIntroduzione: La Sindrome di Poland è una malformazione congenita rara caratterizzata da difetti unilaterali della regione toraco-mammaria, associati in modo variabile ad anomalie ipsilaterali dell’arto superiore. In questa gli aspetti estetici, sociali e di benessere psicofisico non sono trascurabili. La diagnosi precoce e la tempistica dell'approccio terapeutico hanno un impatto positivo sulla qualità di vita. Obiettivi: Questa tesi ha lo scopo di indagare variazioni della qualità di vita pre operatori confrontati a quelli post ricostruzione mammaria. Materiali e metodi: Sono state arruolate 28 pazienti di sesso femminile affette da Sindrome di Poland che risultavano aver completato la ricostruzione mammaria, tra il 2012 e il 2019, mediante l’utilizzo di impianti protesici ed innesto di tessuto adiposo autologo. È stato utilizzato come strumento di analisi il Breast-Q con 8 moduli per la valutazione della soddisfazione e della qualità di vita pre e post-operatoria. Il criterio di esclusione era la ricostruzione mammaria basata su lembi miocutanei o muscolari. Risultati: Gli item 1 (benessere psicosociale), 2 (benessere sessuale) e 6 (benessere fisico-torace) del Breast Q che prevedono valutazioni pre e postoperatoria hanno rilevato una differenza statisticamente significativa per gli item 1 e 2 (p<0.001), border-line (p=0.052) per l’item 6. Non ci sono punteggi migliori per chi ha deformità più lievi (T1B1N1 + T1B1N2) rispetto a tutti gli altri. La suddivisione in due gruppi in base all’età di completamento dell’iter ricostruttivo ( ≤ 20 anni, > 20 anni) non ha rilevato differenza. Conclusioni: La ricostruzione mammaria ha un impatto evidente come outcome di miglioramento di qualità di vita, indipendentemente dalla gravità della malformazione e dall’età in cui la paziente affronta l’intervento chirurgico.it_IT
dc.description.abstractIntroduction: Poland Syndrome is a rare congenital malformation characterized by unilateral defects of the thoraco-mammary region, associated in a variable manner with ipsilateral anomalies of the upper limb. In this, the aesthetic, social and psychophysical well-being aspects are not negligible. Early diagnosis and timing of the therapeutic approach have a positive impact on the quality of life. Objectives: This thesis aims to investigate pre-operative quality of life variations compared to those after breast reconstruction. Materials and methods: 28 female patients with Poland Syndrome were enrolled who resulted to have completed breast reconstruction, between 2012 and 2019, through the use of prosthetic implants and grafting of autologous adipose tissue. The Breast-Q with 8 modules for the assessment of satisfaction and quality of life pre and post-operative was used as the analysis tool. The exclusion criterion was breast reconstruction based on myocutaneous or muscle flaps. Results: Breast Q items 1 (psychosocial well-being), 2 (sexual well-being) and 6 (physical well-being-chest) involving pre- and post-operative assessments found a statistically significant difference for items 1 and 2 (p <0.001), while border-line (p = 0.052) for item 6. There are no better scores for those with milder deformities (T1B1N1 + T1B1N2) with respect to others. In additiont, the division into two groups based on the age of completion of the reconstructive process (≤ 20 years,> 20 years) did not reveal any difference. Conclusions: Breast reconstruction has an evident impact as a quality of life improvement outcome, regardless of the severity of the malformation and the age at which the patient undergoes surgery.en_UK
