dc.description.abstractBackground Sistema nervoso centrale, scheletro e tessuto adiposo sono connessi dall’azione delle adipochine; la relazione tra adiponectina sierica (AS) e densità minerale ossea (BMD) è dubbia. Obiettivi Valutare la relazione tra AS e BMD in lungo sopravviventi da tumore cerebrale pediatrico (CO-BTS) e se influenzata dal grado di disfunzione ipotalamo-ipofisaria (DI-P). Metodi In 103 CO-BTS sono stati valutati dati antropometrici e biochimici e densitometrici mediante DXA 5,9±3,3 anni dopo l’off-therapy ad un’età di14,7±3,5 anni. Una DI-P lieve (assi ipotalamo-ipofisari coinvolti<3) era presente in 62 (LL) mentre una forma severa in 41(assi >3, HL). Valori DXA di Massa Grassa (MG), Magra (MM) e BMD sono stati misurati al rachide lombare (L) e al corpo intero (TB). Risultati M e F non differivano per età, altezza e BMI. Le F avevano minor MM (26,8±8,4 vs31,6±11,1kg, P<0,01) e maggior MG (21,5±10,1 vs18,5±8,7 kg). Soggetti HL erano di età maggiore rispetto ai LL (15,6±3,9 vs13,9±2,5yrs, P<0,01) e avevano BMI, MM, MG maggiori (rispettivamente 1,6±1,7vs0,7±1,2SDS; 33,3±9,2vs26,5±9,8kg; 24,4±11,0vs17,0±7,1 kg, p<0,0001). L e TB-BMD Z-score erano nella norma in F/M e HL/LL, ma ridotti in soggetti LL rispetto ad HL (rispettivamente: -1,1±1,2vs-0,6±1,3 e -1,0±1,0vs-0,4±1,2 P<0,01). L’AS non differiva nei diversi gruppi, tuttavia era inversamente correlata a misure DXA nelle F (-0,28<r<-0,42 P<0,05), ma non nei M, e negli HL (-0,33<r<-0,53, P<0,01), ma non negli LL. Analisi di regressione multivariata mostravano che l’AS predice negativamente il TB-BMD Z-score (β-stand -0,218, P=0,04; R20,41), anche dopo correzione per altezza (p<0,05), MM, MG, insulina e IGF1(p=ns), nei LL, ma non negli HL. Conclusioni L’AS è inversamente correlata a parametri DXA, indipendentemente da adiposità e da altri determinanti di massa ossea, in soggetti con danno ipotalamico lieve/assente. Ciò supporta l’ipotesi di un effetto pro-osteoclastico ipotalamo-mediato di tale ormone sul metabolismo osseo.it_IT
dc.description.abstractBackground Brain, bone and fat are connected through adipokines; controversial associations have been reported between serum adiponectin (AS) and bone mineral density (BMD). Objective To determine the relationship between AS and BMD in childhood-onset brain tumor survivors (CO-BTS); and if it is influenced by hypothalamic-pituitary (h-p) dysfunction. Methods Anthropometrics and biochemical tests were assessed in 103, 14,7±3,5yrs old, 5,9±3,3yrs after off cancer therapy. A low-likelihood (LL) h-p involvement (hormone defects<3), was present in 62 and a high-likelihood (HL) in 41. Lean mass (LM), Fat Mass (FM), bone mineral content and BMD at lumbar spine (LS) and Total Body-less-head (TB) were measured by DXA. BMAD was calculated to adjust BMD for body size. Results F and M did not differ for age, height, BMI yet females had less LM (26,8±8,4 vs31,6±11,1kg, P<0,01) and greater FM than males (21,5±10,1 vs18,5±8,7 kg,P<0,05). HL were older than LL patients, had greater BMI, LM, FM (respectively 1,6±1,7 vs0,7±1,2SDS; 33,3±9,2 vs26,5±9,8kg; 24,4±11,0 vs17,0±7,1 kg all p<0,0001). LS-BMD and TB-BMD Z-scores were similarly reduced in M and F and lower in LL compared to HL patients (-1,1±1,2vs-0,6±1,3-P=ns- and -1,0±1,0vs-0,4±1,2, P<0,01, respectively). AS levels were not different between groups (M/F, LL/HL); however, it was inversely related to DXA parameters in F (-0,28<r<-0,42 all P’s<0,05) but not in M, and in HL (-0,33<r<-0,53, all P’s<0,01) but not in LL. Regression analyses showed that AS inversely and significantly predicted (β-coeff -0,218 P=0,04) TBBMD Z-score value after accounting for HtSDS (P<0,0001), FM, LM, insulin and IGF-1 in LL patients (R2 0,41), but not in HL in which LM was the only predictor. Conclusions AS is inversely related to DXA bone measures in CO-BTS with preserved h-p function, independently of adiposity and determinants of bone, supporting the hypothesis of a central and pro-resorption role of AS on bone homeostasis.en_UK
