dc.description.abstract | Introduzione e Scopo dello studio: Il Rachitismo ipocalcemico carenziale (RC) è patologia pediatrica antica che mostra nuova diffusione, recenti scoperte genetiche hanno incluso forme ipofosforemiche (RI). Scopo dello studio è stato valutare: 1) la distribuzione di RC e RI; 2) le caratteristiche cliniche, biochimiche, radiologiche nel tempo e in base al trattamento, di una coorte monocentrica con diagnosi di rachitismo (R.).
Metodi: dati clinici, biochimici, radiologici e terapeutici sono stati raccolti retrospettivamente in 31 soggetti (20M,11F) con diagnosi di R. tra il 2000 e il 2021: RC in 18, R. secondario ad altra patologia-RS in 5, RI in 7 e R. vitamina D dipendente in 1. Intervalli rilevanti per il follow-up sono stati: diagnosi-DNG, + 1, + 3, + 6, + 9-12, + 24 mesi e ultimo follow up (F-U).
Risultati: Il 74% delle diagnosi di R. è avvenuta tra il 2015-2021, il 19% tra 2010 e 2014 e il 6% in precedenza. Il R. era prevalente nei M (65%), in base al fototipo il R si distribuiva in: intermedio (39%), chiaro (32%), e nero (29%). L’HTSDS media alla DGN era di -1.5±1.7 (n=27), dopo un F.U di 2,9±2,4 anni rimanevano più bassi i RS rispetto ai RC e soggetti con diagnosi di R ipocalcemico >2 anni (-1.7 ± 2.4, =10) rispetto a soggetti < 2 anni (0.4±0.8,n=11). Anche la calcemia rimaneva inferiore nei soggetti >2 anni nel tempo (p<0.01). Dopo un F-U di 3,9±4,6 nella corte intera, rimanevano alterati livelli di ALP e/o PTH nel 50-62% dei soggetti.
Conclusioni: lo studio pilota ha evidenziato un aumento della prevalenza di rachitismo, forme più severe e più resistenti al trattamento sembrano i RS e/o R. insorgenti >2 anni di età. Lo studio del metabolismo ca/p ha dimostrato permanenza di lievi alterazioni in tutte le tipologie di R., in particolare in RI. Fattori che contribuiscono a questa apparente nuova diffusione del rachitismo sono la scoperta di ‘’nuove’’ forme ipofosfatemiche, la società multietnica e una maggiore attenzione al metabolismo ca/p . | it_IT |
dc.description.abstract | Background and aims. Hypocalcemia-related rickets (RC) is an old pediatric disease that is showing a new spread. New Genetics discoveries have included Hypophosphatemia-related rickets (RI). Aims: to evaluate 1) RC and RI distribution; 2) clinical, biochemical, radiological characteristics of a monocentric cohort rickets patients (R), over time and based on treatment.
Methods. We retrospectively collected the clinical, biochemical, radiologic and therapeutic data in 31 patients with R in the period of 2000-2021: 18 with RC, 5 with R secondary to other diseases (RS), 7 with RI and 1 with Vitamin D-dependent R. Intervals relevant to the follow-up were: diagnosis-DNG, +1, +3, +6, + 9-12, +24 months, and the last follow-up (F-U).
Results. 74% of R. diagnoses occurred between 2015-2021, 19% 2010-2014, and 6% previously.
R. prevalence was higher in males (65%). Based on phototype, R distribution is divided in intermediate (39%), bright (32%), and black (29%). Mean age at diagnosis is 4.5±1.3 years, with a difference among R. types at different age (RC 75%, RI 12,5%, RS 6.2% 2 years old vs 40,3, 33.3 e 26.7% >2 years old, p<0.0001). HTSDS was -1.5±1.7 (n=27) at DNG, after a 2,9±2,4 F-U years, patients suffering from RS were shorter than those afflicted by RC or hypocalcemia-related R in >2 years old (-1.7 ± 2.4, =10), compared to <2 years old (0.4±0.8,n=11). Serum calcium remains lower in subjects >2 years over time (p<0,01). After a 3,9±4,6 F-U, ALP and/or PTH levels kept to be modified in the whole cohort.
Conclusions. The pilot study showed a progressive increase in rickets prevalence. RS and/or outbreaking > 2 years old appear to be more severe, and more resistant to treatment. Ca/P metabolism study showed persistence of slight alterations in all R., in particular in the RI. A greater pediatric attention to Ca/P metabolism, the discovery of RI, and the multiethnic society should be considered contributing factors to this apparent new spread of rickets. | en_UK |
