Valutazione della morfologia del palato in pazienti trattati con Leaf Expander ed Espansore Rapido: analisi geometrica morfometrica

Abstract

Scopo: Valutare i cambiamenti della morfologia palatale in pazienti trattati con due diversi dispositivi attraverso l’analisi morfometrica geometrica(GMA), che permette di rappresentare i risultati direttamente con immagini 3D che forniscono informazioni sui cambiamenti di forma. Materiali e Metodi: Nello studio sono stati inclusi 250 pazienti, di entrambi i sessi e tutti con contrazione palatale, divisi in due gruppi: il primo comprendeva 127 pazienti trattati con Espansore Rapido (GrREP), il secondo comprendeva 123 pazienti trattati con Leaf Expander(GrLE). I dispositivi sono stati ancorati sia sugli E sia sui VI e le scansioni dei modelli sono state acquisite sia in fase di pretrattamento(T0) sia dopo 9-11 mesi dalla cementazione del dispositivo(T1). Sono state eseguite la sovrapposizione Procustea e l’analisi delle componenti principali per confrontare i cambiamenti di forma del palato pre e post trattamento. Risultati: La relazione tra il tipo di ancoraggio e la forma della volta palatina al tempo T1 è risultata statisticamente significativa in favore dell’ancoraggio sugli E, che ha miglior esito sulla volta palatina, in entrambi i gruppi. A parità di espansione, né il tipo di trattamento (REP o LE) né il tipo di ancoraggio (E o VI) ha modificato la quantità di espansione ottenuta sui molari. Entrambi i gruppi hanno mostrato un cambiamento significativo da T0 a T1 in senso trasversale con allargamento della volta palatina ma solamente il GrREP ha mostrato differenze in senso verticale, con abbassamento della volta palatina. Conclusioni: La GMA ha mostrato aspetti che non sono visibili dalle analisi tradizionali. A parità di espansione, il REP è risultato migliore del LEAF riducendo l’altezza della volta palatina e l’ancoraggio su E, sia nel GrREP sia nel GrLE, è risultato più efficace nel mutare la forma della volta palatina.

Aim: To evaluate changes in palatal morphology in patients treated with two different devices through geometric morphometric analysis (GMA), which allows the results to be represented directly with 3D images that provide information on shape changes. Materials and Methods: The study included 250 patients, of both sexes and all with palatal contraction, divided into two groups: the first included 127 patients treated with Quick Expander (GrREP), the second included 123 patients treated with Leaf Expander (GrLE). The devices were anchored both on the E and on the VI and the scans of the models were acquired both in the pretreatment phase (T0) and after 9-11 months from the cementation of the device (T1). The Procustea superimposition and the analysis of the main components were performed to compare the changes in shape of the palate before and after treatment. Results: The relationship between the type of anchorage and the shape of the palatine vault at time T1 was statistically significant in favor of the anchorage on the E, which has a better outcome on the palatine vault, in both groups. With the same expansion, neither the type of treatment (REP or LE) nor the type of anchorage (E or VI) has changed the amount of expansion obtained on the molars. Both groups showed a significant change from T0 to T1 in the transverse direction with widening of the palatine vault but only the GrREP showed differences in the vertical direction, with lowering of the palatine vault. Conclusions: GMA has shown aspects that are not visible from traditional analyzes. With the same expansion, the REP was better than the LEAF by reducing the height of the palatine vault and the anchoring on E, both in the GrREP and in the GrLE, was more effective in changing the shape of the palatine vault.
