Valutazione clinica del riassorbimento osseo a 6 mesi dopo grande rialzo del seno mascellare: una revisione sistematica

dc.description.abstract | INTRODUZIONE: La scarsa qualità ossea e la rapida pneumatizzazione del seno nel mascellare posteriore rendono più difficile l’ottenimento di una buona stabilità implantare primaria comportando l’impiego di tecniche chirurgiche rigenerative quali il rialzo di seno. SCOPO DEL LAVORO: La presente revisione sistematica si pone come obiettivo la valutazione del riassorbimento osseo dopo 6 mesi conseguente all’impiego di differenti biomateriali nel grande rialzo di seno mascellare. MATERIALI E METODI: È stata eseguita, da Aprile ad Agosto 2020, una ricerca telematica su tre principali banche dati. Due revisori hanno selezionato solo studi randomizzati controllati (RCT) che confrontassero biomateriali differenti, con un periodo di controllo di almeno 6 mesi ed un minimo di 5 pazienti trattati. Per ogni biomateriale è stato valutato il rimaneggiamento volumetrico, il fallimento implantare e le complicanze post-operatorie. Il risk of bias è stato valutato in base ai criteri Cochrane. RISULTATI: Nella seguente revisione sono stati considerati 11 RCT sui 4010 articoli di partenza, per un totale di circa 302 seni e 248 pazienti analizzati; solo 2 pubblicazioni presentano un risk of bias complessivo basso. Tramite la CBCT sono state valutate le variazioni volumetriche degli innesti sottraendo al valore base quello riscontrato dopo 6 mesi dal grande rialzo. I dati sono stati raggruppati in 5 gruppi: innesto autogeno (AU), xenograft (XG), alloplast (AP), allograft (AG), autogeno + alloplast (AU+AP). Dopo 6 mesi, il biomateriale che ha dimostrato la minore riduzione volumetrica è stato lo XG (7,30±15,49%), mentre quello che si è riassorbito maggiormente è stato l’AU (41,71±12,63%). Le uniche due significative differenze, impiegando il t-test non appaiato, sono state trovate tra AU e XG (P=0,009), e tra Ag e XG (P=0,048). CONCLUSIONI: In base al biomateriale ed al volume innestato, il clinico può prevedere l’entità totale del rialzo in modo da poter ottenere la rigenerazione voluta. | it_IT |
dc.description.abstract | INTRODUCTION: Poor bone quality and rapid sinus pneumatization makes more difficult, in the posterior maxilla, to achieve good primary implant stability, resulting in the use of regenerative surgical techniques such as sinus lift. PURPOSE OF THE WORK: This systematic review aims to evaluate bone resorption and material shrinkage of different biomaterials used in the lateral sinus lift, after 6 months from the surgery. MATERIALS AND METHODS: An online research in three main databases was carried out from April to August 2020. Two reviewers selected only randomized controlled trials (RCTs) that compared different biomaterials used in the lateral sinus lift, with a follow-up period of at least 6 months and a minimum of 5 patients treated. For each biomaterial volumetric resorption, implant failure and post-operative complications were valuated. The risk of bias was assessed according to the Cochrane criteria. RESULTS: 11 RCTs from the 4010 starting articles were included, for a total of 302 sinuses and 248 patients analyzed; only 2 publications have a low overall risk of bias. Through the CBCT, the volumetric variations of the grafts were evaluated by subtracting from the base value the one found after 6 months since the surgery. The data were grouped into 5 groups: autogenous graft (AU), xenograft (XG), alloplast (AP), allograft (AG), autogenous + alloplast (AU + AP). After 6 months, the biomaterial that showed the least volume reduction was XG (7.30 ± 15.49%), while the one that was most reabsorbed was AU (41.71 ± 12.63%). The only two significant differences, employing the unpaired t-test, were found between AU and XG (P = 0.009), and between Ag and XG (P = 0.048). CONCLUSIONS: Based on these information, clinician can predict the total extent of the regeneration to achieve successful results. | en_UK |
