Analisi della misurazione dello spessore coroideale in pazienti affetti da glaucoma

Abstract

Background: Il glaucoma ad angolo aperto (OAG) è una neuropatia ottica cronica progressiva con alterazioni morfologiche della testa del nervo ottico e dello strato delle fibre nervose retiniche e conseguente perdita progressiva delle cellule ganglionari retiniche e diminuzione del campo visivo. Obiettivo del nostro studio è stato valutare la riproducibilità intra ed interosservatore delle misurazioni dello spessore della coroide ottenute mediante OCT e la presenza di correlazione tra spessore coroideale maculare e danno funzionale e anatomico causato dall’OAG. Metodi: Sono stati reclutati 20 pazienti (8F, 12M) affetti da OAG. Per ogni paziente è stato scelto l’occhio con il deficit visivo maggiore, sulla base dei valori di Mean Deviation (MD) e Pattern Standard Deviation (PSD) del campo visivo. Le immagini sono state acquisite con lo strumento Swept-Source OCT-A Plex Elite 9000 e analizzate con il software ImageJ. Lo spessore coroideale è stato misurato in cinque punti: a livello della fovea, a 1000 e a 2000 micron da quest’ultima, sia verso il settore nasale che verso quello temporale. Ogni misurazione è stata eseguita da uno stesso osservatore (per 3 volte) e da quattro osservatori diversi. Risultati: Il grado di riproducibilità intraosservatore è stato più alto di quello interosservatore, risultando in un COV di 2,86 (DS:0,49) e di 11,51 (DS:1,28), rispettivamente. E’ stata valutata una possibile associazione tra spessore coroideale maculare e i marcatori di severità del glaucoma, quali gli indici perimetrici (MD, PSD, sensibilità dell’emicampo superiore ed inferiore), RNFL e GCL+. Tra questi, è emersa una correlazione lineare positiva tra spessore coroideale maculare e MD (r = 0,48). Conclusioni: E’ emersa una correlazione lineare positiva tra spessore coroideale maculare e MD. Il grado di ripetibilità intraosservatore è stato più alto rispetto a quello della riproducibilità interosservatore.

Background: The open angle glaucoma (OAG) is a progressive cronic optic neuropathy characterized by morphological alterations of the optic nerve head and the retinal nerve fibre layer resulting in loss of visual field. The aim of our study has been to evaluate intra and interobserver reproducibility of the choroidal thickness measurements obtained by OCT and the presence of correlation among macular choroidal thickness and functional and anatomical damage caused by the OAG. Methods: 20 patients (8F, 12M) with OAG were recruited. For each patient, the eye with the greatest visual impairment was chosen, based on the mean deviation (MD) and pattern standard deviation (PSD) values of the visual field. The images were acquired by the Swept-Source OCT-A Plex Elite 9000 instrument and analyzed with the ImageJ software. The choroidal thickness was measured in five points: at the fovea, at 1000 and 2000 microns from the latter, both towards the nasal and temporal sectors. Each measurement was performed 3 times by the same observer and by four different observers. Results: The degree of intraobserver reproducibility was higher than the interobserver one, resulting in a COV of 2.86 (DS: 0.49) and 11.51 (DS: 1.28), respectively. A possible association between macular choroidal thickness and glaucoma severity markers, such as perimetric indices (MD, PSD, sensitivity of the upper and lower half-field), RNFL and GCL + was assessed. Among these, a positive linear correlation between macular choroidal thickness and MD emerged (r = 0.48). Conclusions: A positive linear correlation between macular choroidal thickness and MD has emerged. The degree of intraobserver repeatability was higher than interobserver reproducibility.
