dc.description.abstract | INTRODUZIONE
L’obiettivo dello studio è valutare i principali fattori e descrivere le strategie di ossigenoterapia adottate che possano predire un peggiore outcome di malattia nei pazienti affetti da COVID-19.
MATERIALE E METODI
Tale studio osservazionale retrospettivo monocentrico analizza i pazienti afferiti al DEA del Policlinico San Martino nel periodo febbraio-maggio 2020 con tampone positivo per infezione da SARS-CoV-2. Si è proceduto pertanto ad esaminare la relazione tra outcome sfavorevole e i seguenti parametri: caratteristiche generali, parametri emodinamici, fattori di rischio cardio-vascolare, comorbidità ed esami di laboratorio. Gli EGA arteriosi sono stati analizzati al tempo 0, durante ossigenoterapia convenzionale e durante CPAP.
RISULTATI
Dei 780 pazienti considerati l’età media era di 71,9 (+/-15) anni, di cui il 59% uomini.
Nei pazienti con cardiopatia da FE<50% e patologie polmonari restrittive si è osservato un maggior rischio di outcome sfavorevole.
Dall’analisi dell’EGA, i seguenti dati correlavano con outcome sfavorevole:
• SatO2<94%: al tempo 0, prima della CPAP e durante la CPAP;
• rapporto PaO2/FiO2<300 mmHg: al tempo 0, prima della CPAP e durante la CPAP;
• intervalli di PaO2<60 mmHg e PaO2>100 mmHg: al tempo 0 e prima della CPAP.
CONCLUSIONI
Analizzando i parametri vitali, si è osservata una stabilità emodinamica in termini di PA, confermando nell’ipossia la causa principale di morte da COVID-19, differentemente da quanto accade nello shock settico.
I pazienti con cardiopatia da FE<50% e patologie restrittive polmonari hanno presentato un aumento dell’outcome sfavorevole, in quanto, entrambe le comorbidità, riducono la DO2 (oxygen delivery).
I valori di PaO2 permettono di ridurre il rischio di outcome sfavorevole allorquando siano ricompresi tra 60-100 mmHg, evidenziando che i danni da iperossia devono essere tenuti in considerazione e che l’ossigenoterapia nei pazienti Covid-19 deve garantire una PaO2 all’interno di siffatto intervallo | it_IT |
dc.description.abstract | INTRODUCTION
The objective of this study was to evaluate variables and to describe the oxygen supply strategies used in Covid-19 patients that may predict adverse outcomes.
METHODS
This work is a retrospective observational study on all patients admitted to the Emergency Department at the University Hospital of Genoa during the period February-May 2020, having a reverse transcription of polymerase chain reaction (RT-PCR) positive for Sarv-Cov-2 to a nasopharyngeal swab. General features, hemodynamic parameters, cardiovascular risk factors, comorbidities and laboratory tests were analyzed and compared with adverse outcomes. ABG were collected at time 0, on conventional oxygen therapy and on CPAP.
RESULTS
780 patients were found and considered for the study. 59% of patients were male, overall mean age was 71.9 (+/-15).
In addition, in patients having cardiovascular disease (mainly with EF < 50%) and restricted lung disease, an increased risk of adverse outcome was observed.
Considering EGA analysis, variables correlated with adverse outcomes were:
• SatO2 <94%: at time 0, before CPAP and during CPAP;
• PaO2/FiO2 ratio <300 mmHg: at time 0, before CPAP and during CPAP;
• range of PaO2<60 mmHg and PaO2>100 mmHg: at time 0 and before CPAP.
CONCLUSION
Looking at vital parameters in those with adverse outcome we observed hemodynamic stability in terms of blood pressure, but hypoxia was relevant as the main cause of death from COVID-19 diffrently from septic shock cases.
Patients with cardiomiopathy with EF < 50% and restricted lung disease had an increased risk of adverse outcome due to reduction of DO2 (oxygen delivery) caused by both comorbidities.
When PaO2 values were included between 60-100 mmHg range, the risk of adverse outcome decreased, proving that hyperoxia may have a negative effect on outcome comparable to hypoxia, which entails that a correct oxygen supply approach should keep PaO2 values within physiological range. | en_UK |
