dc.description.abstract | Introduzione
Nelle donne postmenopausali con carcinoma mammario operato ormonosensibile la terapia con anti-aromatasici diminuisce il rischio di recidiva e di morte. Tuttavia, l’uso di anti-aromatasici è associato al rischio di eventi scheletrici e cardiovascolari.
Il presente progetto ha valutato la correlazione tra gli SNPs del gene CYP19 e l’efficacia e tossicità della terapia con anti-aromatasici in donne in postmenopausa arruolate nello studio clinico GIM4.
Metodi
Lo studio GIM4 ha randomizzato 2056 pazienti pretrattate con 2-3 anni di tamoxifene a ricevere 2-3 anni o 5 anni (terapia “extended”) di terapia adiuvante con l’anti-aromatisico letrozolo.
4 SNP su diversi loci del gene CYP19: rs4646, rs10046, rs749292 e rs727479 sono stati analizzati con PCR su DNA germinale e le pazienti monitorate ogni 6 mesi negli anni 1-5, annualmente negli anni 5-10.
Risultati
Di 2056 pazienti arruolate nello studio GIM4, 647 sono state sottoposte ad analisi degli SNP.
Nessuno degli SNP è risultato associato al rischio di recidiva e di morte da carcinoma mammario.
La variante allelica CC dello SNP rs10046 e la variante GG in rs727479 sono risultate associate ad aumentata probabilità di eventi scheletrici (OR 2,1; 95% CI 1,3-3,6; p= 0,004, e OR 2,1; 95% CI 1,1-3,8; p= 0,03) ed eventi cardiovascolari (OR 2,2; 95% CI 1,3-3,6; p=0,002, e OR 2,3; 95% CI 1,2-4,1; p=0,006). Le pazienti con genotipo rs10046-CC/rs272479-GG hanno mostrato un aumento del 20% di tali eventi, con test di interazione significativo tra genotipo e trattamento “extended” (p=0.03).
Conclusioni
I risultati mostrano che le pazienti trattate con letrozolo e con rs10046-CC e rs272479-GG hanno un rischio aumentato di eventi scheletrici e cardiovascolari. Il genotipo rs10046-CC/rs272479-GG è un potenziale biomarcatore di tossicità con letrozolo “extended”. | it_IT |
dc.description.abstract | Background
In hormone receptor-positive early breast cancer (eBC), adjuvant aromatase inhibitors decrease the risk of relapse and death. However, such therapy is associated with side effects, including skeletal and cardiovascular events.
This project aims to assess if single nucleotide polymorphisms (SNPs) in the gene encoding the aromatase enzyme (CYP19) could affect the efficacy and safety of aromatase inihbitors therapy in postmenopausal women with eBC.
Methods
This project has been conducted on patients enrolled in the GIM4 trial, which randomized 2056 postmenopausal eBC patients previously treated with 2-3 years of tamoxifene to receive either 3-2 years or 5 years (extended arm) of adjuvant letrozole.
Four SNPs (rs10046, rs4646, rs727479, rs 749292) at different loci of CYP19 were genotyped through allele-specific PCR amplification from patients’ constitutional DNA. Patients got regular checkups every 6 months in the first 5 years and then every year until the 10th year .
Results
Of 2056 patients enrolled in the GIM4 trial, 647 have available data on SNPs.
We found no significant correlation between SNPs and disease-free survival.
The allelic variants CC of rs10046 and GG of rs727479 were significantly associated to higher risk of skeletal events (OR 2.1; 95% CI 1.3-3.6; p=0.00 and OR 2.1; 95% CI 1.1-3.8; p=0.03, respectively) and cardiovascular events (OR 2.2; 95% CI 1.3-3.6; p=0.002 and OR 2.3; 95% CI 1.2-4.1; p=0.006, respectively). Patients with genotype rs10046-CC/rs272479-GG treated with 5 years letrozole showed a 20% increase in the risk of skeletal and cardiovascular events as compared with genotype-matched patients treated with 2-3 years letrozole (interaction test p=0.03).
Conclusions
Patients with rs10046-CC and rs272479-GG have a significant higher risk of skeletal and cardiovascular events upon adjuvant letrozole. Genotype rs10046-CC/rs272479-GG could be a useful biomarker of increased toxicity with exteded letrozole therapy. | en_UK |
