La "changing-face" del Servizio di Trasporto di Emergenza Neonatale in Italia, durante la pandemia COVID-19

Abstract

Scopo: Valutare l’impatto della pandemia Covid-19 sul Servizio di Trasporto di Emergenza Neonatale (STEN) (periodo marzo-dicembre 2020) e confrontarlo con la precedente survey nazionale del 2018. Materiali e metodi: Survey costituita da questionario di 20 domande rivolto ai 53 STEN attivi in Italia: volume annuale di attività, tipo di organizzazione, politiche per la valutazione della qualità, formazione e istruzione, veicoli utilizzati. Nello specifico sono stati conteggiati per ogni singolo STEN attivo il numero di trasporti di neonati di madre SARS-Cov2 positiva, neonati e lattanti SARS-Cov2 positivi. Risultati: I principali risultati della survey 2021 (periodo marzo-dicembre 2020) utili per lo scopo dello studio hanno evidenziato un totale di 5679 trasporti neonatali dei quali 3804 trasporti primari, 813 trasporti terziari e 462 back-transport. Del totale dei trasporti neonatali effettuati 248 erano relativi a figli di madre Covid positiva, 20 erano neonati Covid positivi e 6 lattanti Covid positivi. Il Neonatal Transport Index (NTI) è pertanto risultato pari a 1,37% per tutti i trasporti, mentre pari a 27,4% relativo alle gravidanze Covid correlate (nati 2020, marzo-dicembre 334860; gravidanze covid correlate 976). Conclusioni: L’analisi generale dei dati 2020 versus 2018 dimostra che il NTI è rimasto sostanzialmente invariato (1,37% vs 1,39% rispettivamente) ampliamente compreso nel range di normalità (1-2%). Se invece consideriamo solo le gravidanze Covid correlate notiamo che fino al 27,4% di queste hanno necessitato del trasferimento del neonato presso un centro di livello superiore. Riteniamo che questo dato non sia dovuto a un fallimento delle cure perinatali relative alla pandemia, ma che piuttosto sia dovuto alla centralizzazione delle gravidanze Covid positive privilegiando l’aspetto materno e trascurando inevitabilmente l’aspetto neonatale.

Purpose: To evaluate the impact of the Covid-19 pandemic on the Neonatal Emergency Transport Service (NETS) (period March-December 2020) and compare it with the previous national survey of 2018. Materials and methods: Survey consisting of a questionnaire of 20 questions addressed to the 53 NETS active in Italy: annual volume of activities, type of organization, policies for quality assessment, training and education, vehicles used. Specifically, the number of transports of SARS-Cov2 positive mothers, SARS-Cov2 positive newborns and infants were counted for each single active NETS. Results: The main results of the 2021 survey (period March-December 2020) useful for the purpose of the study highlighted a total of 5679 neonatal transports of which 3804 primary transports, 813 tertiary transports and 462 back-transports. Of the total neonatal transports carried out, 248 were related to children of a positive Covid mother, 20 were Covid positive newborns and 6 Covid positive infants. The Neonatal Transport Index (NTI) was 1.37% for all transport, while was 27.4% for Covid-related pregnancies (born 2020, March-December 334860; covid-related pregnancies 976). Conclusions: The general analysis of 2020 versus 2018 data shows that the NTI remained substantially constant (1.37% vs 1,39% respectively) widely included in the normal range (1-2%). If, on the other hand, we consider only the related Covid pregnancies, we note that up to 27.4% of these required the transfer of the newborn to a higher level center. We believe that this figure is not due to a failure of perinatal care related to the pandemic, but rather that it is due to the centralization of positive Covid pregnancies, favoring the maternal aspect and inevitably neglecting the neonatal aspect.
