Uso compassionevole dei farmaci: le approvazioni del Comitato Etico della Regione Liguria nel triennio 2018-2020

L’uso compassionevole dei farmaci sta diventando una pratica medica sempre più diffusa; esso permette a pazienti affetti da malattie gravi o rare di allargare lo spettro di terapie a disposizione potendo usufruire di farmaci in fase di sperimentazione, che ancora non hanno ottenuto l’approvazione da parte delle autorità sanitarie. Lo scopo di questa tesi è di effettuare un’analisi delle richieste di farmaci in uso compassionevole pervenute al Comitato Etico della Regione Liguria nel triennio 2018-2020. Le richieste per singolo anno sono state suddivise per ente richiedente e in base alla categoria di appartenenza del farmaco. Per ogni anno è stata individuata la patologia per la quale sono state avanzate il maggior numero di richieste. Si è effettuata in seguito un’analisi comparativa tra i singoli anni, analizzando se ci sia stato un cambiamento nella distribuzione. Nel periodo 2018-2020 il Comitato Etico della Regione Liguria ha ricevuto richieste di uso compassionevole per 439 pazienti. Nell’ ultimo anno si è osservato un incremento della domanda di farmaci in uso compassionevole rispetto al primo anno del 187% e ciò è legato alla pandemia da Covid19. In genere, la maggior parte delle richieste riguardava pazienti oncologici non responsivi alle terapie standard. Nell’anno 2020 si è verificato un incremento della domanda di usi compassionevoli in ambito infettivo, che in termini assoluti ha avuto una tendenza a equiparare le richieste in ambito oncologico. I risultati mostrano come l’uso compassionevole dei farmaci rappresenti una parte fondamentale della pratica terapeutica. L’incremento della domanda osservato negli anni mette in risalto la velocità con cui nuovi farmaci vengono messi a punto e immessi sul mercato nel tentativo di migliorare le opportunità terapeutiche per i pazienti e denota una sempre maggiore consapevolezza da parte dei medici di questa opzione terapeutica.

The compassionate use of drugs is becoming an increasingly widespread medical practice. It allows patients suffering from serious or rare diseases to have a larger range of available therapies, including drugs in the trial phase which have not yet obtained approval from the health authorities. The purpose of this thesis is to carry out an analysis of the requests for compassionate use drugs received by the Ethics Committee of the Liguria Region in the three-year period 2018-2020. The requests for each year were divided by requesting institution and by the category to which the drug belongs. For each year it was identified the pathology for which the greatest number of requests were made. A comparative analysis was then carried out between the individual years, analyzing whether there has been a change in the distribution. In the period 2018-2020, the Ethics Committee of the Liguria Region received requests for compassionate use for 439 patients. In the last year there has been an increase in the demand for drugs in compassionate use compared to the first year of 187% and this increment is explained by the Covid19 pandemic. Typically, most of the requests were made for cancer patients unresponsive to standard therapies. In the year 2020 there was an increase in the demand for compassionate uses in the infectious field, which in absolute terms has tended to equalize the requests in the oncology field. The results show that the compassionate use of drugs represents a fundamental part of the therapeutic practice. The increase in demand observed over the years highlights the speed with which new drugs are developed and placed on the market in an attempt to improve therapeutic opportunities for patients and denotes an increasing awareness of this therapeutic option on the doctors’ side.
