dc.description.abstract | BACKGROUND:
La telemedicina, e in particolare il telemonitoraggio, per il paziente con patologie croniche, è vista sempre di più come strumento essenziale per rendere più compliante la terapia ed efficiente il trattamento medico.
OBIETTIVO:
L’obiettivo di questa tesi è duplice:
1) Valutare l’efficacia e l’efficienza di interventi di telemedicina sui percorsi di monitoraggio delle patologie croniche, tramite gli studi di costo-efficacia presenti in letteratura.
2) Esaminare se, negli studi pubblicati, i dati forniti dai vari trial e meta-analisi visti siano sufficienti per una valutazione completa dei vantaggi delle prestazioni di telemedicina e come debbano essere più correttamente integrati.
METODI:
E’ stata fatta una rassegna di diversi studi in cui veniva eseguita l’analisi CEA di interventi di telemonitoraggio in quattro patologie croniche: Diabete, Asma, Bpco, Ipertensione. Ho cercato dagli anni 2000 al 2021 in un Database, PubMed, i lavori inclusi sono stati 30 su un totale di 397 e ho esaminato gli studi in base ai criteri di ammissibilità, poi ho confrontato i risultati.
RISULTATI:
I risultati della valutazione economica sono apparsi piuttosto eterogenei e si sono esaminate le cause delle difformità riscontrate analizzando sia i costi che i benefici degli interventi di telemonitoraggio effettuati. I risultati finali sono stati positivi e dimostrano che la telemedicina è costo-efficace.
CONCLUSIONE:
La telemedicina porta un lieve miglioramento rispetto alla cura tradizionale, ma gli studi esaminati risultano spesso piuttosto incompleti e dovrebbero essere standardizzati, in particolare le voci di costo, per poter procedere ad un’analisi economica accurata.

Telemedicine, and in particular telemonitoring, for the patient with chronic diseases, is increasingly seen as an essential tool to make therapy more compliant and medical treatment more efficient.
OBJECTIVE:
The objective of this thesis is twofold:
1) To evaluate the effectiveness and efficiency of telemedicine interventions on chronic disease monitoring pathways, through cost-effectiveness studies in the literature.
2) To examine whether, in the published studies, the data provided by the various trials and meta-analyses seen are sufficient for a comprehensive assessment of the benefits of telemedicine services and how they should be more properly integrated.
METHODS:
A review was made of several studies in which CEA analysis of telemonitoring interventions in four chronic conditions was performed: Diabetes, Asthma, Bpco, Hypertension. I searched from the years 2000 to 2021 in a Database, PubMed, the included papers were 30 out of a total of 397 and reviewed the studies according to the eligibility criteria, then compared the results.
RESULTS:
The results of the economic evaluation appeared rather heterogeneous and the causes of the discrepancies found were examined by analysing both the costs and benefits of the telemonitoring interventions performed. The final results were positive and demonstrate that telemedicine is cost-effective.
CONCLUSION:
Telemedicine brings a slight improvement over traditional care, but the studies reviewed are often rather incomplete and should be standardised, particularly the cost items, in order to perform an accurate economic analysis.
