dc.description.abstract | Introduzione Nel dicembre 2019 è emerso a Wuhan (Cina) un nuovo coronavirus umano. A differenza dei precedenti coronavirus SARS-CoV-1 e MERS-CoV, il nuovo SARS-CoV-2, agente eziologico del COVID-19, si è diffuso in tutto il mondo generando una pandemia. L'apparato più frequentemente e gravemente colpito da SARS-CoV-2 è quello respiratorio, ma sono state riportate anche manifestazioni neurologiche, sia in acuto che in cronico (facenti parte del "long-COVID"). Studi autoptici hanno ritrovato SARS-CoV-2 nell'encefalo di pazienti deceduti a causa del COVID-19 e si ipotizza che il virus possa raggiungere il sistema nervoso centrale risalendo il nervo olfattivo oppure seguendo la via ematogena. Metodi Abbiamo sottoposto un questionario per valutare astenia, depressione e deficit mnesici a due popolazioni di pazienti guariti dal COVID-19. Popolazione 1: soggetti senza alterazioni neurologiche. 111 pazienti, di cui 69 maschi (62,2%) e 42 femmine (37,8%). Età media 60,9 anni (range 33-92). Popolazione 2: soggetti con alterazioni neurologiche ed inviati a visita neurologica. 61 pazienti, di cui 32 maschi (52,4%) e 29 femmine (47,6%). Età media 59,8 anni (range 19-81). Risultati Il punteggio medio del questionario è risultato significativamente più alto nella popolazione con alterazioni neurologiche rispetto a quella senza (14,36 vs 8,4; p=0,00002). Non abbiamo riscontrato significative correlazioni tra punteggio del questionario ed età o sesso (con eccezione per la scala dell'astenia, di valore significativamente più alto nelle femmine: 9,93 vs 7,18; p=0,027). Il COVID-19 si è presentato più gravemente nei maschi (p=0,001) e nei soggetti ≥ 60 anni (p=0,027). Conclusioni Astenia e deficit mnesici sono più spesso riportati da pazienti con alterazioni neurologiche rispetto a quelli senza. L'astenia ha un impatto maggiore sulla popolazione femminile. Il nostro questionario potrebbe essere un valido strumento per individuare i soggetti meritevoli di un follow-up neurologico. | it_IT |
dc.description.abstract | Introduction In December 2019, a new human coronavirus emerged in Wuhan (China). Unlike of the previous coronaviruses SARS-CoV-1 and MERS-CoV, the new SARS-CoV-2, etiological agent of COVID-19, spread around the world generating a pandemic. The system most frequently and severely affected by SARS-CoV-2 is the respiratory system, but neurological manifestations have also been reported, both acute and chronic (part of the "long-COVID"). Autopsy studies have found SARS-CoV-2 in the brains of patients who died from COVID-19 and it is hypothesized that the virus can reach the central nervous system by going back up the olfactory nerve or following the hematogenous route. Methods We submitted a questionnaire to evaluate asthenia, depression and memory deficits to two patient populations recovered from COVID-19. Population 1: subjects without neurological alterations. 111 patients, including 69 males (62.2%) and 42 females (37.8%). Mean age 60.9 years (range 33-92). Population 2: subjects with neurological alterations and sent for neurological examination. 61 patients, of which 32 males (52.4%) and 29 females (47.6%). Mean age 59.8 years (range 19-81). Results The mean questionnaire score was significantly higher in the population with neurological alterations compared to that without (14.36 vs 8.4; p=0.00002). We found no significant correlation between questionnaire score and age or sex (with the exception of the asthenia scale, of value significantly higher in females: 9.93 vs 7.18; p=0.027). COVID-19 occurred more severely in males (p=0.001) and in subjects ≥ 60 years (p=0.027). Conclusions Asthenia and memory deficits are more often reported by patients with neurological alterations than those without. Fatigue has a greater impact on the female population. Our questionnaire can be a valid tool to identify subjects deserving of a neurological follow-up. | en_UK |
