Disease Management Team (DMT) per i disturbi cognitivi: ruolo della Genetica Medica nella gestione di pazienti complessi.

Abstract

This thesis analyzes the function of the Disease Management Team (DMT-CCDD Center for Cognitive Disorders and Dementia) in the management of complex cases, with a special focus on the role that the specialist in Medical Genetics plays within multidisciplinary teams. All cases of patients diagnosed with a disease of genetic etiology presented to date in the context of the DMT-CCDD were examined starting from the first of these, held on 07/26/2017. Five of these identified cases were selected as examples of how the Medical Genetics, within the DMT-CCDD, acts and performs its functions in the routine clinical practice. The advantages in the management of challenging patients by means of the DMT-CCDD highlighted in this dissertation were the following: - the multidisciplinary evaluation allows diagnostic insights that open to unexpected scenarios, such as a diagnosis of a rare and hereditary disorder that has implications, as well as clinical and therapeutic on the patient, also on his family; - the DMT proves to be an effective tool in quickly directing the patient towards the correct diagnosis given the plurality of figures involved and the possibility of plenary discussion; - the possibility for the experts of the DMT-CCDD to draw up or modify PTDA and clinical protocols in the light of the LL.GG. specific for the pathologies of the patients under examination, in the event that there are "gray areas" not attributable to existing guidelines. The cases herein discussed demonstrate in which way the participation of medical geneticists within the DMT-CCDD is essential for the management of complex patients by sharing specific clinical experience with all the specialists belonging to the DMT-CCDD. In conclusion, it is evident that the inclusion of Medical Genetics in a multidisciplinary context brings a benefit to the patient in terms of time and quality of care and, moreover, allows the taking into care not only of the patient but of his entire family.

In questa tesi si analizza la funzione del Disease Management Team (DMT-CDCD Centro Disordini Cognitivi e Demenze) nella gestione di casi complessi, con particolare attenzione al ruolo che lo specialista in Genetica Medica riveste all’interno dei team multidisciplinari. Sono stati presi in esame tutti i casi dei pazienti con diagnosi di malattia a eziologia genetica presentati fino ad oggi nel contesto del DMT-CDCD a partire dal primo di questi, tenutosi il 26/07/2017. Tra i casi analizzati ne sono stati selezionati cinque, esemplificativi di come la Genetica Medica, all’interno del DMT-CDCD, agisca e svolga le sue funzioni nella pratica clinica. I vantaggi nella gestione dei pazienti complessi per mezzo del DMT-CDCD evidenziati in questa tesi sono stati i seguenti: - la valutazione multidisciplinare permette approfondimenti diagnostici che aprono a scenari inattesi, quali una diagnosi di patologia rara ed ereditaria che ha delle ricadute, oltre che cliniche e terapeutiche sul paziente, anche sulla sua famiglia; - il DMT si dimostra uno strumento efficace nell’indirizzare rapidamente il paziente verso la diagnosi corretta vista la pluralità di figure coinvolte e la possibilità di discussione plenaria; - la possibilità da parte dei professionisti del DMT-CDCD di redigere o modificare PTDA e protocolli clinici alla luce delle LL.GG. specifiche per le patologie dei pazienti in esame, nel caso in cui ci siano delle “zone grigie” non ascrivibili a linee guida esistenti. I casi analizzati dimostrano come la presenza dei genetisti medici all’interno del DMT-CDCD sia fondamentale per la gestione dei pazienti complessi condividendo l’esperienza clinica specifica con tutti gli specialisti facenti parte del DMT-CDCD. In conclusione, è evidente come l’inserimento della Genetica Medica in un ambito multidisciplinare porti un beneficio per il paziente in termini di tempi e di qualità di cura e, inoltre, permetta la presa in cura non solo del paziente ma di tutta la sua famiglia
