dc.description.abstract | Introduzione: il ritardo o restrizione di crescita intrauterina (IUGR) nei neonati pretermine con peso inferiore ai 1500 grammi (VLBW) è noto essere condizione aggravante la prognosi neurologica a distanza, sebbene non è chiaro come si distribuiscano le più tipiche lesioni della prematurità, specie quando diagnosticate con RM cerebrale.
Obiettivi: valutare l’incidenza di lesioni cerebrali nei VLBW con diagnosi di IUGR e di SGA (Small for Gestational Age, SGA, peso inferiore al decimo percentile) rispetto ai nati VLBW con peso appropriato per età (AGA). Correlare tali dati con outcome neurologico a distanza.
Metodi: tutti i pretermine VLBW nati tra il 1-1-12 e il 31-12-16 presso l’Istituto G. Gaslini e sottoposti a RM cerebrale alla età corretta di 40 settimane sono stati inclusi e valutati mediante scale di sviluppo Griffiths al compimento dei 36 mesi di vita. Le lesioni considerate sono state: GMH (Germinal Matrix Haemorrhage), IVH (Intraventricular Haemorrhage), CBH (Cerebellar Haemorrhage), Micro-CBH (micro-cerebellar Hemorrhage), PWML (Punctate White Matter Lesions) e PVHI (Periventricular Hemorrhagic Infarction).
Risultati: di 352 VLBW, 91 risultavano IUGR (25,6%) e 47 di questi risultavano anche SGA (13,3%). I nati IUGR presentavano una maggiore incidenza di GMH (41,8% vs 17,6% p<0,05), IVH (36,3% vs 18% p<0,05), CBH (25,3% vs 15,7% p<0,05) e micro-CBH (25,3% vs 12,4% p<0,05). I nati IUGR non SGA presentavano una maggiore incidenza di GMH (52,3% vs 19,9% p<0,05), IVH (50% vs 18,6 p<0,05), CBH (29,5% vs 16,9% p<0,05) e microCBH (27,3% vs 14,1% p<0,05). In 12 SGA senza diagnosi prenatale di IUGR non era presente questo aumento dell’incidenza. IUGR non SGA presentavano più bassi punteggi alla Scala Griffiths a 2 e 3 anni e questa diminuzione non era presente nei nati SGA non IUGR.
Conclusioni: la condizione di IUGR sembra presentare un rischio maggiore di sviluppare lesioni emorragiche e potrebbe avere effetti sull’outcome neurologico. | it_IT |
dc.description.abstract | Introduction: intrauterine growth retardation or restriction (IUGR) in preterm newborns weighing less than 1500 g (VLBW) is known to be an aggravating condition for long-term neurological prognosis, although it is not clear how the most typical prematurity lesions are distributed, especially when diagnosed with brain MRI.
Aim: assess the incidence of brain lesions in VLBW with a prenatal diagnosis of IUGR and SGA (Small for Gestational Age, weight under the tenth percentile) compared to VLBW appropriate for gestational age (AGA).
Methods: all preterms under 1500 g (VLBW) born between 1-1-12 and 31-12-16 at Gaslini Children’s Hospital that were subjected to brain MRI at 40 weeks, were included in the study and tracked during time with Griffiths Scale. Brain lesions considered were: GMH (Germinal Matrix Haemorrhage), IVH (Intraventricular Haemorrhage), CBH (Cerebellar Haemorrhage), Micro-CBH (micro-cerebellar Hemorrhage), PWML (Punctate White Matter Lesions) and PVHI (Periventricular Hemorrhagic Infarction).
Results: among 352 VLBW, 91 were IUGR (25,6%) and 47 of them where also SGA (13,3%). IUGR newborns showed an increased incidence of GMH (41,8% vs 17,6% p<0,050), IVH (36,3% vs 18% p<0,05), CBH (25,3% vs 15,7% p<0,05) and micro CBH (25,3% vs 12,4% p<0,05). IUGR non-SGA had a higher incidence of GMH (52,3% vs 19,9% p<0,05), IVH (50% vs 18,6 p<0,05), CBH (29,5% vs 16,9% p<0,05) and microCBH (27,3% vs 14,1% p<0,05). In 12 SGA babies without the prenatal diagnosis of IUGR this increase of incidence was not present. IUGR non-SGA showed lower scores at Griffiths Scale at 2 and 3 years and this decrease was not present in SGA non IUGR.
Conclusions: IUGR condition seems to present a higher risk to develop hemorrhagic lesions and it may affect neurodevelopment. | en_UK |
