| dc.description.abstract | Introduzione. | it_IT |
| dc.description.abstract | Background We aimed to investigate pathologic complete response (pCR) and major pathologic response (MPR) as potential surrogate endpoints for event-free survival (EFS) and overall survival (OS) in early-stage non-small cell lung cancer (NSCLC) chemoimmunotherapy trials. Methods A systematic literature review of randomized neoadjuvant/perioperative chemoimmunotherapy trials was carried out. Using reconstructed individual patient-level data and a two-stage meta-analytic model, we assessed pCR and MPR as surrogate endpoints by examining correlations with EFS and OS at the outcome level and correlations of treatment effects at the trial level. Results Of 6005 records screened, 8 trials were included, comprising 3247 patients. At the outcome level, among 6 trials (n=2700), patients with pCR (n=358, 13.3%) had lower risk of an EFS event than patients without pCR (HR 0.17, 95% CI 0.12-0.23, p<0.0001). At the trial level, among 8 trials, a weak correlation was observed between treatment effects on pCR and EFS (R2 0.30, 95% CI 0.00-0.63). Similar results were obtained for OS at the outcome level (HR 0.11, 95% CI 0.04-0.28, p<0.0001) and trial level (R2 0.40, 95% CI 0.00-0.71). At the outcome level, among 4 trials (n=1874), patients with MPR (n=470, 25.1%) had lower risk of an EFS event than patients without MPR (HR 0.22, 95% CI 0.17-0.29, p<0.0001). At the trial level, among 8 trials, a moderate correlation was observed between treatment effects on MPR and EFS (R2 0.54, 95% CI 0.00-0.76). Similar results were obtained for OS at the outcome level (HR 0.25, 95% CI 0.14-0.43, p<0.0001) and trial level (R2 0.57, 95% CI 0.00-0.79). Conclusions Our results indicate that, despite better outcomes in patients with pCR, pCR and MPR are insufficiently correlated with survival endpoints at the trial level to be considered fully validated surrogate endpoints in chemoimmunotherapy trials for early-stage NSCLC. | en_UK |
