| it_IT |
| dc.description.abstract | Acute aortic syndromes (AAS) are time-dependent conditions with high mortality that require prompt diagnosis and treatment in highly specialized centers. This study analyzes the experience of a regional HUB center in the management of these emergencies.
The study is ambispective, monocentric, and observational, conducted between 2020- 2025 in patients who presented directly to the HUB or were transferred from spoke hospitals within the regional network. Both patients who underwent surgical intervention (IC) and those treated with medical therapy alone (TM) were analyzed.
A total of 55 consecutive patients with a mean age of 67 ± 14 years were enrolled: 37 (67,3%) were transferred from spoke centers and 18 (32,7%) presented directly to the HUB. The IC group included 31 patients (56,4%), while 24 (43,6%) received TM. In the IC group, 7 patients (22,6%) were treated in the hyperacute phase, 20 (64,5%) in the acute phase, and 4 (12,9%) in the subacute phase. Mean time from symptom onset to treatment was 6,3 ± 4.0 days in the acute phase and 31 ± 16,2 days in the subacute phase. Indications for treatment were visceral/peripheral malperfusion (35,5%), high-risk radiological criteria (38,7%), persistent pain (16,1%), and rupture (9,7%). The approach was exclusively endovascular in 22 cases (71%) and hybrid in 9 (29%). At 30 days, 4 deaths (12.9%) occurred in the IC group (2 in the hyperacute phase and 1 in the acute phase), while no deaths occurred in the TM group. Mean follow-up was 33,1 ± 19,4 months; estimated 48-month survival was 64,7% in the IC group versus 81,6% in the TM group (p=0.166). Freedom from reintervention was 71,6% in the IC group, while freedom from intervention was 68,3% in the TM group.
Centralization at a regional HUB center ensures effective management, a multidisciplinary approach, and appropriate selection of endovascular and surgical therapies, confirming the validity of the HUB-Spoke organizational network. | en_UK |
