Risultati: Sono state analizzate 598 pazienti per complessivi 1.082 cicli: 126 appartenenti a POSEIDON 3 (21,1%) e 472 a POSEIDON 4 (78,9%).
Tuttavia, il guadagno cumulativo è marcatamente superiore nelle donne under 35, confermando l'età ovocitaria come fattore prognostico determinante. | it_IT |
| dc.description.abstract | Introduction: In infertility treatment using ART (Assisted Reproductive Technology), patients with diminished ovarian reserve constitute a heterogeneous group with a poor prognosis; the POSEIDON classification stratifies them based on age and AMH levels. However, the true clinical benefit of performing repeated cycles on the cumulative live birth rate (CLBR) remains debated in the literature.
Objectives: To evaluate the trends of the Cumulative Pregnancy Rate (CPR) and Cumulative Live Birth Rate (CLBR) up to three consecutive oocyte pick-up cycles in POSEIDON groups 3 and 4.
Materials and Methods: Monocentric retrospective study (2015–2025, FRU Center San Martino, Genoa); the unit of analysis is the oocyte pick-up (OPU) cycle. Cumulative estimates were obtained using Kaplan-Meier analysis; the association with the POSEIDON group was evaluated using a Cox proportional hazards model (Hazard Ratio, 95% CI).
Results: A total of 598 patients were analyzed for a total of 1,082 cycles: 126 belonging to POSEIDON 3 (21.1%) and 472 to POSEIDON 4 (78.9%). Over the first three cycles, CPR and CLBR increased progressively in both groups. The cumulative CPR increased from 15.9% to 37.8% and 55.9% in group 3 vs. 14.6%, 24.8%, and 35.5% in group 4. The cumulative CLBR rose from 13.5% to 33% and 52.1% in POSEIDON 3, compared to 10.8%, 17%, and 22.3% in POSEIDON 4. In the Cox model, the POSEIDON 4 group showed a significantly lower probability of both pregnancy and live birth compared to group 3.
Conclusions: In patients with diminished ovarian reserve, reproductive potential is not exhausted with the first pick-up but continues to increase up to the third cycle. However, the cumulative gain is markedly higher in women under 35, confirming oocyte age as a determining prognostic factor. A limitation of this study is the pessimistic estimation of the CLBR, as all patients who do not undergo a subsequent cycle after the first are categorized as negative outcomes. | en_UK |
