| it_IT |
| dc.description.abstract | INTRODUCTION: Thoracic trauma accounts for approximately 15% of all traumatic events and contributes to 25% of trauma-related mortality. The prospective, multicentre observational study “TRACTIONS” introduced the Lung Ultrasound Score (LUS) as a tool for the early quantification of pulmonary injury by means of ultrasonography.OBJECTIVES: To evaluate the ability of the LUS to anticipate clinical deterioration in patients with thoracic trauma.MATERIALS AND METHODS: This study included adult patients with thoracic trauma and a GCS >8 who were managed in the ED of Policlinico San Martino between February 2023 and April 2026.The LUS was assessed from admission through day 7, with monitoring for the occurrence of complications, mortality, transfer to the Intensive Care Unit (ICU), or the need for ventilatory support. The “MAXLUS” value was recorded, defined as the highest LUS value detected during the observation period.RESULTS: A total of 83 patients were enrolled (75.9% male and 24.1% female), with a mean age of 58.3 ± 18.7 years. Eleven patients (13.3%) experienced an unfavorable clinical course, characterized by the occurrence of at least one severe complication. The mean total LUS was 1.66 ± 1.6 among patients who developed complications and 2.67 ± 3.2 among those without complications. In 44.6% of cases, complete assessment of all pulmonary fields was precluded by the presence of dressings. The MAXLUS value was significantly associated with age, the number of rib fractures, and the presence of haemothorax and/or pneumothorax; conversely, no association was observed between MAXLUS and the occurrence of unfavorable clinical outcomes.DISCUSSION AND CONCLUSIONS: Serial LUS assessment appears to be a valuable tool for the initial assessment and monitoring of the evolution of pulmonary contusions. However its clinical applicability may be hampered by the presence of dressings and extraparenchymal lesions which can interfere with its prognostic utility. | en_UK |
