| dc.description.abstract | BACKGROUND: Le donne BRCA mutate presentano un aumentato rischio di carcinoma mammario e ovarico. Rimangono da chiarire le implicazioni prognostiche legate a tipo e localizzazione delle varianti patogenetiche (PVs) nei geni BRCA1/2 in pazienti con carcinoma ovarico.
MATERIALI: BRCA BCY Collaboration è uno studio di coorte retrospettivo, multicentrico e internazionale, coinvolgente 109 centri. In questa analisi sono state incluse donne con PVs dei geni BRCA e carcinoma mammario in stadio I–III ≤40 anni con successivo carcinoma ovarico. L’endpoint primario era la sopravvivenza globale (OS) in base a tipo di variante (INDEL inserzione-delezione, SNV variante singolo nucleotide, CNV variazione numero copie), proteina tronca, non tronca e tipo di conseguenza sulla codifica.
RISULTATI: Su 5660 pazienti, 71 hanno sviluppato carcinoma ovarico. Il follow-up mediano era 5,3 anni. Il 45,07% delle pazienti presentava variante INDEL, 42,25% SNV e 12,68% CNV. Il 61,97% aveva PV tronca e 7,04% non tronca. Le mutazioni frameshift erano le più comuni (45,16%), seguite da nonsenso (25,81%), splicing (20,97%) e missenso (8,06%).
Non è stata osservata alcuna associazione significativa tra tipo di variante e OS, ma un trend a favore della variante SNV rispetto a CNV (HR 1,93 IC 95% 0,50-7,50) e INDEL (HR 2,04 IC 95% 0,83-5,04). Non sono emerse differenze significative in OS tra proteina tronca e non tronca (HR 1,03 IC 95% 0,14-7,83). L’analisi in base al tipo di conseguenza sulla codifica ha mostrato che pazienti con mutazioni frameshift hanno un trend in OS peggiore rispetto agli altri tipi di mutazione (HR missenso 0,73 IC 95% 0,10-5,65; HR nonsenso 0,41 IC 95% 0,13-1,22; HR splicing 0,41 IC 95% 0,12-1,45).
CONCLUSIONI: Questa analisi non ha dimostrato differenze significative in termini di OS in base alle PVs dei geni BRCA1/2 in pazienti con carcinoma ovarico dopo pregresso carcinoma mammario.
Ulteriori studi sono necessari per chiarire l’effetto dei diversi difetti nei geni BRCA1/2. | it_IT |
| dc.description.abstract | BACKGROUND: BRCA-mutated women have an increased risk of breast and ovarian cancer. The prognostic implications, linked to type and location of pathogenetic variants (PVs) in the genes BRCA1/2 in patients with ovarian cancer, remain to be clarified.
MATERIALS: BRCA BCY Collaboration is a retrospective, multicenter and international cohort study, involving 109 centers. In this analysis, women with PVs of BRCA genes and stage I–III breast cancer ≤40 years, with a subsequent ovarian cancer, were included. The primary endpoint was overall survival (OS) according to variant type (INDEL insertion-deletion, SNV single nucleotide variant, CNV copy number variation), truncated, non-truncated protein and type of coding consequence.
RESULTS: Out of 5660 patients, 71 developed ovarian cancer. Median follow-up was 5,3 years. 45,07% of patients had INDEL variants, 42,25% SNV, and 12,68% CNV. 61,97% had truncating PVs and 7,04% non-truncating. Frameshift mutations were most common (45,16%), followed by nonsense (25,81%), splicing (20,97%), and missense (8,06%). No significant association was observed between variant type and OS, but a trend favoring SNV over CNV (HR 1,93 95% CI 0,50-7,50) and INDEL (HR 2,04 95% CI 0,83-5,04). No significant differences emerged in OS between truncated and non-truncated protein (HR 1,03 95% CI 0,14-7,83). Analysis by type of coding consequence showed that patients with frameshift mutations have a worse OS trend compared to other mutation types (missense HR 0,73 95% CI 0,10-5,65; nonsense HR 0,41 95% CI 0,13-1,22; splicing HR 0,41 95% CI 0,12-1,45).
CONCLUSIONS: This analysis showed no significant differences in OS based on BRCA1/2 gene PVs in patients with ovarian cancer after prior breast cancer.
Further studies are needed to clarify the effect of different defects in BRCA1/2 genes. | en_UK |
