Abstract

Introduzione e Scopo dello Studio Le patologie autoimmuni e le immunodeficienze rappresentano poli opposti del disequilibrio immunitario. Lo studio mira a esplorare differenze tra pazienti con patologie autoimmuni e con immunodeficienze, e le loro manifestazioni gastroenterologiche, per individuare biomarcatori utili alla diagnosi precoce e differenziale. Metodi È stato condotto uno studio osservazionale monocentrico di 12 mesi presso l’Ospedale San Bartolomeo di Sarzana. Sono stati analizzati 76 pazienti con età > 18 anni (26 con patologie gastroenterologiche e 50 con immunodeficienze). Si sono poi valutati dodici biomarcatori infiammatori e immunologici (VES, PCR, ferritina, IgG, IgA, IgM, C3, C4, ALT, PLT, Hb, creatinina), analizzati con test non parametrici (Mann-Whitney U, Fisher) e correzione per false discovery rate (FDR). Risultati Lo studio ha evidenziato un’età significativamente più elevata nei pazienti con immunodeficienze rispetto a quelli gastroenterologici. Pur senza differenze statisticamente significative nei biomarcatori, sono emersi trend coerenti: VES, ALT e ferritina più alti nei soggetti gastroenterologici e livelli di IgG inferiori nei pazienti immunodeficienti, in linea con la fisiopatologia attesa. Conclusioni Lo studio suggerisce la potenziale utilità dei biomarcatori di routine nello screening differenziale tra patologie gastrointestinali e immunodeficienze, fornendo basi per futuri studi multicentrici mirati a sviluppare profili diagnostici più accurati.

Introduction and Aim of the Study Autoimmune diseases and immunodeficiencies represent opposite poles of immune imbalance. This study aimed to explore differences between patients with autoimmune and immunodeficiency disorders, focusing on their gastroenterological manifestations, in order to identify potential biomarkers useful for early and differential diagnosis. Methods A 12-month, single-center observational study was conducted at San Bartolomeo Hospital in Sarzana. Seventy-six patients aged over 18 years were analyzed (26 with gastroenterological disorders and 50 with immunodeficiencies). Twelve inflammatory and immunological biomarkers (ESR, CRP, ferritin, IgG, IgA, IgM, C3, C4, ALT, PLT, Hb, creatinine) were evaluated using non-parametric tests (Mann-Whitney U, Fisher) with correction for false discovery rate (FDR). Results The study revealed a significantly higher median age in patients with immunodeficiencies compared to those with gastroenterological diseases. Although no statistically significant differences were found in the biomarkers, consistent trends emerged: higher ESR, ALT, and ferritin levels in gastroenterological patients, and lower IgG levels in immunodeficient subjects, consistent with the expected pathophysiological profiles. Conclusions The study highlights the potential usefulness of routine biomarkers as differential screening tools between gastrointestinal diseases and immunodeficiencies, laying the groundwork for future multicenter studies aimed at developing more accurate diagnostic profiles.
