Confronto degli effetti dento-scheletrici dell'espansione mascellare con Leaf Expander ed Espansore Rapido del Palato: studio multicentrico randomizzato

Abstract

OBIETTIVO: Confrontare gli effetti dentali e scheletrici di tre dispositivi per l’espansione mascellare (Espansore Rapido del Palato, Leaf Expander 450 grammi, Leaf Expander 900 grammi) con uno studio prospettico controllato e randomizzato condotto in tre centri. MATERIALI E METODI: Sono stati selezionati 103 bambini con ipoplasia del mascellare superiore e randomizzati in tre gruppi: RPE (n=32, età media 8,6±1,5 anni), Leaf 450 (n= 38, età media 8,4±1,8 anni) e Leaf 900 (n=33, età media 8,1±1,6 anni). Il tempo medio osservazione è stato di 1,1±0,3 anni. I cambiamenti pre- e post-trattamento sono stati valutati su modelli digitali e teleradiografie in proiezione postero-anteriori. RISULTATI: Tutti i gruppi hanno mostrato incrementi significativi nelle misure trasversali e angolari a livello dei molari a T1. Il confronto tra gruppi ha evidenziato differenze significative nel perimetro d'arcata tra il gruppo Leaf 450 ed il gruppo Leaf 900 (3,6 mm vs 5,5 mm, p=0,012), e tra il gruppo Leaf 450 ed il gruppo RPE (3,6 mm vs 5,4 mm, p<0,001). Differenze significative sono state individuate nella derotazione dei molari superiori tra il gruppo Leaf 450 e RPE (-5,1° vs -2,5°, p<0,001) e tra Leaf 900 e RPE (-4,4° vs -2,5°, p<0,001). L’analisi delle teleradiografie ha mostrato un aumento significativamente maggiore dell'ampiezza lateronasale nel gruppo RPE rispetto ai gruppi Leaf 450 e Leaf 900. (3,2 mm vs 1,9 mm vs 2,2 mm, ANOVA p=0,012). CONCLUSIONI: I risultati di questo studio confermano che tutti i dispositivi analizzati si sono dimostrati efficaci nel determinare un’espansione trasversale significativa e modificazioni dento-alveolari e scheletriche nei pazienti in crescita. I soggetti trattati con Leaf Expander hanno beneficiato di una maggiore derotazione dei molari superiori, mentre il perimetro d’arcata è aumentato maggiormente nei soggetti trattati con Leaf 900 ed RPE. L’RPE ha determinato un incremento significativamente maggiore dell'ampiezza lateronasale.

AIM: To compare the dental and skeletal effects of three maxillary expansion devices (Rapid Palatal Expander, Leaf Expander 450 grams, Leaf Expander 900 grams) using a prospective controlled and randomized study conducted across three centers. MATERIALS AND METHODS: A total of 103 children with maxillary hypoplasia were selected and randomized into three groups: RPE (n=32, mean age 8.6±1.5 years), Leaf 450 (n=38, mean age 8.4±1.8 years), and Leaf 900 (n=33, mean age 8.1±1.6 years). The average observation period was 1.1±0.3 years. Pre- and post-treatment changes were evaluated on digital models and posteroanterior cephalometric radiographs. RESULTS: All groups showed significant increases in transverse and angular measurements at the molar level at T1. The group comparisons revealed significant differences in the arch perimeter between the Leaf 450 and Leaf 900 groups (3.6 mm vs 5.5 mm, p=0.012) and between the Leaf 450 and RPE groups (3.6 mm vs 5.4 mm, p<0.001). Significant differences in molar derotation were found between Leaf 450 and RPE (-5.1° vs -2.5°, p<0.001) and between Leaf 900 and RPE (-4.4° vs -2.5°, p<0.001). Cephalometric analysis of the posteroanterior radiographs showed a significantly greater increase in the lateronasal width in the RPE group compared to the Leaf 450 and Leaf 900 groups (3.2 mm vs 1.9 mm vs 2.2 mm, ANOVA p=0.012). CONCLUSIONS: The results of this study confirm that all the devices analyzed were effective in achieving significant transverse expansion and dento-alveolar and skeletal changes in growing patients. Subjects treated with the Leaf Expander experienced greater derotation of the upper molars, while the arch perimeter increased more significantly in the Leaf 900 and RPE groups. The RPE group showed a significantly greater increase in the lateronasal width.
