dc.description.abstractNel 2015 è stato creato il primo set di criteri per la classificazione della sindrome di Behçet (BS) pediatrica, i criteri PEDBD.Obiettivi: validare i criteri PEDBD in una coorte esterna di pazienti.Metodi:210 pazienti (70BS,40 PFAPA,35 FMF,26 MKD,22 TRAPS,17 UND/ SURF) sono stati selezionati in maniera casuale da Eurofever (esclusione se >16 anni e inclusi nello studio PEDBD).11 Esperti internazionali sono stati scelti per valutare i pazienti: nel 1° round sono stati mostrati i dati clinici e sierologici, nel 2° sono stati aggiunti i dati genetici e nel 3° i commenti e voti degli altri esperti.Usando il consenso degli Esperti come gold standard (consenso>80%),sono stati applicati i criteri PEDBD, ISG e ICBD alla coorte di pazienti BS ed alle malattie confondenti al fine di calcolarne sensibilità, specificità ed accuratezza.Risultati: al termine del 3° round è stata raggiunta una consensus sulla diagnosi iniziale in 112/210 pazienti (53.3%), con un consenso ulteriore su 27 pazienti cui è stata assegnata una diagnosi finale differente.I 24 BS con consenso sono stati definiti “BS confermati”, mentre i 10 con consenso nel 60-70% degli esperti come “BS probabili”.Comparando questi pazienti con la coorte di malattie confondenti, gli elementi distintivi del BS sono risultati: un’età d’esordio tardiva, la presenza di afte orali, genitali, sintomi cutanei (follicolite/acne), di patergia cutanea e di uveite posteriore.Nei 24 BS-confermati, gli ISG, ICBD e i PEDBD hanno mostrato rispettivamente una sensibilità di 0.50, 0.79 e 0.58, una specificità di 1.00, 0.98, 0.99, ed una accuratezza di 0.90, 0.954 and 0.91.Conclusioni: i PEDBD sono estremamente specifici ma non molto sensibili, gli ICBD hanno avuto una miglior performance.Il BS pediatrico è una malattia ad esordio insidioso, per cui al fine di migliorare i criteri sono necessari studi prospettici con coorti internazionali di grandezza maggiore, che includano anche le differenze etniche ed i cluster di malattia esistenti.it_IT
dc.description.abstractIn the past, several criteria have been createed for Behçet’s syndrome (BS) classification. In 2015, the first set of BS paediatric classification criteria, the PEDBD, was proposed. Aim: to perform an external validation of the PEDBD criteria Methods: 210 patients (70BS, 40 PFAPA, 35 FMF, 26 MKD, 22 TRAPS, 17 UND/ SURF) were randomly selected from the Eurofever Registry (patients excluded if > 16 years and if included in first PEDBD study). A set of 11 Experts blinded to original diagnosis, were chosen to evaluate the patients: in the 1st round, clinical and serological data were evaluated; in the 2nd round genetic data were added; in the 3rd round the other Experts’ votes and comments were shown. Using the expert consensus as gold standard (agreement>80%), the PEDBD, the ISG and the ICBD criteria were applied to BS patients and to the confounding diseases in order to define the sensitivity, specificity and accuracy. Results: At the end of the 3rd round, a consensus on the initial diagnosis was reached in 112/210 patients (53.3%), with an additional consensus with a different final diagnosis in 27 patients. BS patients with an agreement (24) were classified as confirmed-BS, and those with an agreement of 60-70% (10) as probable-BS. When comparing these patients with the confounding diseases, a later onset of BS, the presence of oral and genital ulcers, skin lesions, positive pathergy test and posterior uveitis were BS distinctive elements. In 24 confirmed-BS, ISG, ICBD and PEDBD criteria showed a sensitivity of 0.50, 0.79 and 0.58, a specificity of 1.00, 0.98, 0.99, and an accuracy of 0.90, 0.954 and 0.91, respectively. Conclusions: the PEDBD were extremely specific but they lack sensitivity, while ICBD had a better performance. The complexity of childhood BS suggests larger prospective international cohorts to foster the performance of criteria, and to understand if BS clusters and ethnic variables should be added to the criteriaen_UK
