dc.description.abstractLa diagnosi di pielonefrite acuta (PNA) si basa su dati clinici, biochimici e di imaging. Un corretto inquadramento diagnostico con idoneo trattamento è necessario per evitare lo sviluppo di cicatrici renali con danno renale cronico. Questa tesi riguarda l’analisi in itinere dei dati raccolti ad 11 mesi dall’avvio del Progetto di Ricerca Finalizzata 2019 “RF-2019-12371234” per valutare le correlazioni tra dati clinici-laboratoristici e coinvolgimento del parenchima renale valutato in fase acuta con la risonanza magnetica in sequenza DWI, ripetuta dopo 6 mesi. Tra gennaio e novembre 2022 sono stati arruolati 37 pazienti (età <3 anni) con diagnosi di prima infezione febbrile delle vie urinarie. La raccolta dei dati clinici e laboratoristici è avvenuta all’ingresso in Istituto (T0) e al momento dello shift dalla terapia antibiotica (abt) da parenterale a orale (T1). Entro 72 ore dall’avvio della abt, sono state effettuate ecografia (eco) e RM-DWI renale, per identificare eventuali focolai pielonefritici. A tutti i pazienti arruolati è stato programmato un controllo a 5-8 mesi dall’ evento acuto (T2), ad oggi effettuato in 12/37, con rivalutazione clinica-laboratoristica-strumentale. I pazienti con PNA documentata con RM-DWI (20/37) e con eco (17/37) avevano livelli di PCR più elevati rispetto ai pazienti senza focolai. Questa valutazione è su una piccola parte del campione di studio previsto dal protocollo. I dati preliminari sembrano dimostrare come l’RM-DWI sia una tecnica utile e sicura per identificare la presenza di lesioni pielonefritiche in età pediatrica, anche se è eseguibile solo in centri altamente specializzati e richiede la sedazione del bambino. La prosecuzione dello studio, ampliato con la valutazione del NGAL urinario, marker di danno tubulare, e con la CEUS (ecografia con ecoemplificatore) potrebbe aiutare a identificare, insieme con la ripetizione della RM-DWI, i soggetti a rischio per una possibile evoluzione verso la cronicizzazione del danno renait_IT
dc.description.abstractThe diagnosis of acute pyelonephritis (APN) is based on clinical, biochemical and imaging data. Correct diagnostic framing with appropriate treatment is necessary to avoid the development of renal scarring with chronic kidney damage. This thesis concerns the in-progress analysis of data collected 11 months after the start of the 2019 Finalised Research Project "RF-2019-12371234" to evaluate the correlations between clinical-laboratory data and renal parenchyma involvement assessed in the acute phase with DWI sequence MRI, repeated after 6 months. Between January and November 2022, 37 patients (age <3 years) with a diagnosis of first febrile urinary tract infection were enrolled. Clinical and laboratory data were collected on admission to the Institute (T0) and at the time of the shift from parenteral to oral antibiotic therapy (abt) (T1). Within 72 hours after the start of abt, ultrasound (echo) and renal MRI-DWI were performed to identify any pyelonephritic foci. All enrolled patients were scheduled for a follow-up 5-8 months after the acute event (T2), which has been performed to date in 12/37, with clinical-laboratory-instrumental re-evaluation. Patients with PNA documented with MRI-DWI (20/37) and with echo (17/37) had higher PCR levels than patients without outbreaks. This evaluation is on a small part of the protocol study sample. Preliminary data seem to show that MRI-DWI is a useful and safe technique to identify the presence of pyelonephritic lesions in paediatric age, although it can only be performed in highly specialised centres and requires sedation of the child. The continuation of the study, expanded with the evaluation of urinary NGAL, a marker of tubular damage, and with CEUS (ultrasound with echo-emplification) could help to identify, together with the repetition of MRI-DWI, subjects at risk for a possible evolution towards chronic renal damage.en_UK
