This thesis explores the management of occupational safety through organizational learning and reporting systems, adopting a perspective focused on risk anticipation. The most influential theoretical contributions to safety approaches are illustrated –with specific reference to the frameworks proposed by Reason, Argyris and Schön and Heinrich–, along with the most recent perspectives on learning from incident precursors, with particular attention to the weak signal concept. However, even though the literature recognises its importance for safety improvement, empirical studies on their application, especially in the field of reporting systems, are still limited. Therefore, a study was conducted to address this gap, through the analysis of the content of the reporting system of a large italian hospital, exploring the presence, characteristics and management of weak signals. A sample of 147 reports was analysed through the Content Analysis method (Elo and Kyngäs) and the Reflexive Thematic Analysis method (Braun and Clarke). The results confirm the non-negligible presence of signals in the reporting system (around 38%, considering both strong and weak signals), but show that their informational potential is often lost or not sufficiently exploited: reports are often characterized by concrete descriptions, as well as the reported anticipations, which are present only in a small portion of reports; at the same time, the organizational interventions proposed in response to the reports are frequently absent or limited. These results suggest, on the one hand, some critical issues in the organizational management of this information, and, on the other hand, the possibility of using different tools, better suited to bring out and enhance the specific characteristics of this type of informational resource.en_UK
