The Expectation-Maximization (EM) algorithm is a fundamental tool for maximum likelihood estimation from incomplete data. This thesis follows the algorithm from its theoretical foundations to an application in immunogenetics, developed during an internship at the Italian Bone Marrow Donor Registry (IBMDR). After reviewing maximum likelihood estimation and the limitations of classical methods, the general formulation of the algorithm is presented, together with its monotonicity property and convergence results. The versatility of the method is illustrated through Gaussian mixture models and image reconstruction in emission tomography. The thesis then shows how the EM algorithm makes it possible to estimate HLA haplotype frequencies from genotypes lacking phase information. Using these frequencies, estimated in R from the Italian registry and the main international registries, the probability that an Italian patient finds a fully matched (8/8) unrelated donor at the HLA-A, -B, -C and -DRB1 loci was estimated, generalizing the reference biostatistical model to a joint search across multiple registries. With the current 400,000 donors, the Italian registry alone yields a probability of 30.3%, whereas searching all registries raises it to 58.8%, higher than what would be achieved by making the national registry five times larger. Finally, the theoretical estimate was compared with the probability observed in 11,997 patients listed between 2020 and 2025, obtained with the inverse Kaplan-Meier method: one year after listing, the probability of finding a donor is 57.5%. The closeness of the two values provides a mutual validation of the two approaches and confirms the effectiveness of the EM algorithm for the problem addressed.en_UK
