Efficacia e sicurezza di ocrelizumab a lungo termine in una coorte di pazienti affetti da sclerosi multipla

Abstract
Background: la Sclerosi Multipla (SM) è una malattia infiammatoria cronica del SNC. Il ruolo dei linfociti B ha guidato lo sviluppo di terapie anti-CD20 come Ocrelizumab (OCR), approvato per le forme RMS e PPMS. I dati real-word a lungo termine tuttavia sono ancora limitati. Obiettivo: valutare il profilo di efficacia e sicurezza a lungo termine di OCR. Metodi: studio osservazionale retrospettivo monocentrico su 186 pazienti affetti da SM che hanno iniziato il trattamento con OCR tra 2017 e il 2021 presso il Centro SM dell’IRCCS Ospedale San Martino di Genova. Sono stati raccolti dati clinici, laboratoristici e radiologici al baseline e prima di ogni infusione. L’efficacia è stata valutata tramite ricadute, EDSS, attività RM, PIRA e NEDA-3; la sicurezza tramite eventi infettivi e sviluppo di neoplasie. Risultati: il farmaco ha dimostrato elevata efficacia, con solo 2 pazienti RMS con ricadute cliniche. Nella coorte totale il 15,1% ha mostrato attività in RM, il 33,9% ha sviluppato eventi PIRA e il 54,3% ha preservato il NEDA-3. L’EDSS basale e l'età baseline sono risultati predittori indipendenti significativi di PIRA nella coorte totale. Le stesse analisi sono state condotte anche per le coorti RMS e PPMS. Riguardo la sicurezza, nella coorte totale il 75,3% ha sviluppato infezioni minori, il 36,1% ipogammaglobulinemia (risultata inoltre fattore di rischio indipendente per infezioni minori, soprattutto UTI), il 37,6% linfopenia (associata a riattivazioni HSV). Anche il numero di infusioni si è dimostrato associato all’incremento di infezioni minori (in particolare URTI e HSV). Inoltre, il 15,1% ha sviluppato infezioni severe e il 7,5% una neoplasia. Conclusioni: OCR mostra elevata efficacia e un buon profilo di sicurezza nel lungo termine. Tuttavia, l’associazione tra insorgenza di PIRA ed età avanzata e maggiore disabilità al baseline suggerisce la necessità di un intervento terapeutico precoce e ad alta efficacia.
 
Background: Multiple Sclerosis (MS) is a chronic inflammatory disease of the CNS. The role of B-cells has driven the development of anti-CD20 therapies such as Ocrelizumab (OCR), approved for RMS and PPMS forms. However, long-term real-world data remain limited. Objective: to evaluate the long-term efficacy and safety profile of OCR. Methods: a single-center retrospective observational study was conducted on 186 MS patients who initiated OCR treatment between 2017 and 2021 at the MS Center of the IRCCS Ospedale San Martino in Genoa. Clinical, laboratory, and radiological data were collected at baseline and prior to each infusion. Efficacy was assessed through relapses, EDSS, MRI activity, PIRA, and NEDA-3; safety was evaluated based on infectious events and the development of malignancies. Results: the drug demonstrated high efficacy, with only 2 RMS patients experiencing clinical relapses. In the total cohort, 15.1% showed MRI activity, 33.9% developed PIRA events, and 54.3% maintained NEDA-3 status. Baseline EDSS and baseline age were found to be significant independent predictors of PIRA in the total cohort. The same analyses were also conducted for the RMS and PPMS cohorts. Regarding safety, 75.3% of the total cohort developed minor infections, and 36.1% developed hypogammaglobulinemia (which was also identified as an independent risk factor for minor infections, particularly UTIs). Lymphopenia occurred in 37.6% of patients and was associated with HSV reactivations. The number of infusions was also associated with an increase in minor infections (specifically URTIs and HSV). Furthermore, 15.1% developed severe infections and 7.5% developed a malignancy. Conclusions: OCR shows high efficacy and a favorable long-term safety profile. However, the association between PIRA onset and both advanced age and higher baseline disability suggests the need for early and highly effective therapeutic intervention.
