Malattia di Alzheimer: approccio neuropsicologico e relazioni interpersonali.

Abstract

La malattia di Alzheimer è una delle grandi sfide per la medicina moderna, sia per la sua crescente diffusione sia per l'influenza devastante che depone su pazienti e rispettive famiglie. Questa tesi esamina quattro dimensioni fondamentali della malattia. Il primo concerne le basi anatomo-fisiologiche, con l'analisi dei cambiamenti strutturali e funzionali nel cervello, come la formazione di placche amiloidi e grovigli neuro-fibrillari, e il loro impatto sul funzionamento neuronale. Il secondo aspetto è l'approccio neuropsicologico, che analizza i deficit cognitivi principali correlati alla malattia e la rilevanza di una diagnosi precoce e accurata, efficace per condizionare e plasmare il decorso e la gestione della patologia. Un altro argomento centrale è la comunicazione della diagnosi, affrontando le sfide etiche e pratiche nell’informare sia i pazienti che i loro familiari; vengono esposte le migliori pratiche per metterla in pratica in modo compassionevole, tenendo in considerazione le reazioni emotive e i bisogni informativi. Infine, si indagano le relazioni interpersonali dei pazienti, ponendo l’attenzione sull'influenza della malattia sulle dinamiche familiari e sociali, con particolare interesse ai caregiver. Vengono analizzate le difficoltà che questi contrastano e le strategie di supporto per migliorare la qualità di vita di pazienti e caregiver. In sintesi, questa tesi offre una prospettiva complessiva della malattia di Alzheimer, con il fine documentare le teorie e le prassi attuali in fase diagnostica e di supporto ai pazienti e alle loro famiglie, mediante un’analisi delle basi scientifiche, neuropsicologiche, comunicative e relazionali.

Alzheimer's disease is one of the great challenges for modern medicine, both due to its growing diffusion and the devastating influence it has on patients and their families. This thesis examines four fundamental dimensions of the disease. The first concerns the anatomical-physiological bases, with the analysis of structural and functional changes in the brain, such as the formation of amyloid plaques and neuro-fibrillary tangles, and their impact on neuronal functioning. The second aspect is the neuropsychological approach, which analyzes the main cognitive deficits related to the disease and the relevance of an early and accurate diagnosis, effective in influencing and shaping the course and management of the pathology. Another central topic is the communication of the diagnosis, addressing the ethical and practical challenges in informing both patients and their families; Best practices are exposed for putting it into practice in a compassionate way, taking into account emotional reactions and information needs. Finally, the interpersonal relationships of patients are investigated, focusing on the influence of the disease on family and social dynamics, with particular interest in caregivers. The difficulties they face and the support strategies to improve the quality of life of patients and caregivers are analysed. In summary, this thesis offers an overall perspective of Alzheimer's disease, with the aim of documenting the theories and current practices in the diagnostic and support phase for patients and their families, through an analysis of the scientific, neuropsychological, communicative and relational bases.
