La neuropatia delle piccole fibre (SFN) è caratterizzata da un danno delle fibre nervose mielinizzate Aδ e non mielinizzate C. La biopsia cutanea è il gold standard nella diagnosi di SFN. Speso, i pazienti con SFN o altri tipi di neuropatie lamentano prurito o dolore, ma i meccanismi patologici di questi due sintomi non sono ancora completamente noti. Lo scopo è stato quello di confrontare le strutture neurovascolari nel derma e nel sub-epidermide tra soggetti affetti da neuropatia periferica (PN) con/senza dolore e/o prurito e controlli sani (HC), per rilevare eventuali differenze nei contatti neurovascolari, che potrebbero fornire informazioni sulla fisiopatologia di questi sintomi. Abbiamo arruolato 25 HC e 79 pazienti PN, divisi in 4 gruppi in base ai sintomi. I campioni di cute, prelevati dalla coscia, sono stati colorati con doppia immunofluorescenza (PGP9.5 e CD31). Le immagini acquisite con il microscopio Leica DMi8 "Thunder" sono state analizzate con il software Fiji, utilizzando il tool "Dermal Layer Analysis". Confrontando HC con i gruppi di pazienti con PN con prurito e/o dolore, sono emerse differenze significative tra HC e pazienti con PN e prurito. Nei soggetti con prurito c’è una significatività di (a)area (p=0.046) e densità(p=0.051) dei vasi, (b)numero (p=0.011), area(p=0.007) e densità(p=0.002) delle piccole fibre, (c)numero (p=0.015), area(p=0.002) e densità(p=0.002) dei contatti vascolari nel sub-epidermide. Mostravano valori inferiori di (d)numero (p=0.021), area(p=0.018) e densità(p=0.008) delle piccole fibre, e(e) numero (p=0.012) e area (p=0.023) dei contatti vascolari nel derma. Differenze simili sono state riscontrate anche tra pazienti con PN e prurito rispetto a pazienti con PN e dolore. I pazienti con NP e prurito mostravano il maggior deterioramento delle strutture neurovascolari subepidermiche e dermiche, suggerendo che il prurito è più correlato a un deterioramento dei contatti tra vasi e piccole fibre nella pelle, rispetto al dolore INTRODUCTION: Small fiber neuropathy (SFN) is characterized by selective impairment of myelinated Aδ and unmyelinated C nerve fibers. Skin biopsy is the gold standard in the diagnosis of SFN. Often, patients with SFN or other types of neuropathies complain of itching or pain, but the pathomecanisms of these two symptoms are still not completely known.
AIM OF THE STUDY: Compare neurovascular structures in sub-epidermis and dermis between subjects affected by peripheral neuropathy(PN) with/without pain and/or itch and healthy controls(HC), to detect any differences in neurovascular contacts, which may give information regarding the pathophysiology of these symptoms.
MATERIALS AND METHODS: We enrolled 25 HC and 79 PN patients, divided into 4 groups according to the symptoms. The skin samples, taken from the thigh, were stained with double immunofluorescence(PGP9.5 and CD31). The images acquired with the microscope Leica DMi8 "Thunder" were analyzed with the software Fiji, using the "Dermal Layer Analysis” tool.
RESULTS: Comparing the groups of patients with PN complaining of itch and/or pain with HC, significant differences emerged between HC and patients with PN and itch. Subjects with itch had significantly lower values of (a) vessel area(p=0.046) and density(p=0.051), (b) small fiber number(p=0.011), area(p=0.007), and density(p=0.002), (c) vascular contact number (p=0.015), area(p=0.002), and density(p=0.002) in the sub-epidermis. Moreover, they showed lower values of d) small fiber number(p=0.021), area(p=0.018), and density(p=0.008), and (e) vascular contact number(p=0.012) and area(p=0.023) in the dermis. Similar differences were also found between patients with PN and itch vs patients with PN and pain.
CONCLUSION: Patients with NP and itch showed the greatest impairment in subepidermal and dermal neurovascular structures, suggesting that itch is more correlated with an impairment of the contacts between vessels and small nerve fibers in the skin, than pain.
