dc.description.abstract | Obiettivo: questo studio ha indagato l'accuratezza diagnostica e le caratteristiche dell'endometriosi parametrIale con l'ecografia in pazienti con sospetta endometriosi profonda (EP).
Metodi: è stato uno studio prospettico (ULTRA-PARAMETRENDO III;NCT06017531) che ha coinvolto pazienti programmate per un intervento chirurgico per sospetta EP. Sono state eseguite ecografie preoperatorie secondo i criteri IDEA ed una topografia parametriale ecografica precedentemente sviluppata. La diagnosi chirurgica di EP è stata utilizzata come riferimento.
Risultati: sono stati confermati chirurgicamente noduli endometriosici parametriali in 105 pazienti su 545 (18,5% a sinistra, 17,0% a destra). L'ecografia transvaginale ha mostrato una sensibilità del 77,1% (IC 95% 68,0–84,8%) e una specificità del 99,1% (IC 95% 67,7–99,8%). I noduli parametriali sono stati tipicamente moderatamente ipoecogeni (83,6%), a forma stellata (74,7%), con margini irregolari (70,2%) e scarsa vascolarizzazione. Il parametrio posteriore era la sede più comune (52,2%), seguito da quello laterale (41,0%) e anteriore (6,8%). Tra le pazienti con coinvolgimento parametriale, concomitanti noduli di EP sono stati frequentemente riscontrati nel retto (63,5%) e nel setto rettovaginale (56,5%), maggiormente che in pazienti senza endometriosi parametriale (p<0,001). Ovaie fisse alla parete uterina (71,8%) e l'assenza dello "sliding sign posteriore" (51,8%) sono stati i segni indiretti di EP più comuni nelle pazienti con noduli parametriali (p<0,001). Anche se rara (8,2%), l'idronefrosi è stata ampiamente rilevata in presenza di noduli parametriali laterali (70,0%).
Conclusione: questo è il primo tentativo di caratterizzare sistematicamente le lesioni endometriosiche parametriali attraverso l'ecografia, anche valutando l'accuratezza diagnostica della stessa, in una significativa coorte di pazienti con sospetta EP. | it_IT |
dc.description.abstract | Objective: this study aimed to investigate ultrasonographic diagnostic accuracy and characteristics of parametrial endometriosis comprehensively.
Method: this prospective study (ULTRA-PARAMETRENDO III;NCT06017531) enrolled patients with suspected deep endometriosis (DE) scheduled for surgical treatment. Women underwent preoperative ultrasonographic examinations following the IDEA criteria and a previous ultrasonographic parametrial topography. The presence of parametrial endometriosis and its characteristics at ultrasound were evaluated, with surgical diagnosis serving as the reference standard.
Results: surgical confirmation of parametrial DE nodules was obtained in 105 out of 545 patients (18.5% left, 17.0% right). Transvaginal ultrasound (TVS) exhibited a sensitivity of 77.1% (95%CI 68.0–84.8%) and specificity of 99.1% (95%CI 67.7–99.8%). Parametrial DE nodules typically presented with mild hypoechoic appearance (83.6%), starry morphology (74.7%), irregular margins (70.2%), and low vascularization. The posterior parametrium was the most common location (52.2%), followed by the lateral (41.0%) and anterior parametrium (6.8%). Among patients with parametrial involvement, the most frequently observed concomitant DE nodules on ultrasound were in the rectum (63.5%) and in the rectovaginal septum (56.5%), both significantly more prevalent than in patients without it (p<0.001). Similarly, among indirect signs of DE, ovaries fixed to the uterine wall (71.8%) and the absence of a posterior sliding sign (51.8%) were more common in women with parametrial DE nodules (p<0.001). Although relatively uncommon (8.2%), hydronephrosis was largely detected in the case of lateral parametrial DE nodules (70.0%).
Conclusion: this is the first attempt to provide a ultrasonographic characterization of parametrial DE lesions, comprehensively assessing the diagnostic accuracy of TVS in identifying DE involvement within a sizeable cohort of patients presenting with suspected DE. | en_UK |
