Lunghezza del telomero e citopenie: esperienza monocentrica

Alterazioni di telomero/telomerasi causano le telomeropatie, patologie con caratteristiche diverse, ma accomunate dal precoce accorciamento telomerico.
Obiettivi: Valutare la lunghezza del telomero e la correlazione con aspetti clinici, ematologici e genetici in pazienti con segni clinici di discheratosi congenita (DC) (e familiari) o soggetti con citopenia persistente.
Metodi: La citopenia di una delle tre linee ematopoietiche era definita con i criteri CTCAE 5.0 e detta immunologica se associata ad autoimmunità. Le anomalie fenotipiche valutate con uno score e definite come “lieve”(<6), “moderato”(6-15),e “grave”(>15) a seconda della gravità dei “ sintomi”. I telomeri sono stati misurati con metodica flow FISH. Il pannello NGS comprendeva 156 varianti genetiche responsabili di insufficienza midollare, immunodeficienza.
Risultati Da aprile 2015 a febbraio 2020 nell’unità di Ematologia dell’Istituto G. Gaslini, sono stati raccolti dati di 55 soggetti divisi in 3 gruppi: gr 1 con telomero normale (37/55), gr 2 con telomero tra il 1°-5° percentile (3/55) e gr 3 con telomero < 1° percentile (15/55). Le stimmate ed i geni DC erano significativamente più frequenti nei gruppi 2 e 3 rispetto al gr 1 (p=0.04 p= 0.0009), mentre non si rilevavano differenze per ciò che concerneva la citopenia, l’insufficienza midollare e lo score fenotipico. I linfociti B e le cellule NK erano le classi più frequentemente ridotte in particolare nei gruppi 2 e 3, mentre la citopenia immunologica era più frequentemente associata a geni di disregolazione immunitaria nel gr 1. Buona concordanza si rilevava per le combinazioni anomalie fenotipiche + citopenia, e citopenia + fenotipo DC tra ai gruppi 2 e 3 (k=0.7)
Conclusioni Pz con stimmate di DC meritano lo studio dei telomeri, mentre nelle citopenie immunologiche potrebbe non essere sempre necessario. Lunghezze dei telomeri anche < 5° percentile pongono il “sospetto” di telomeropatie e sono meritevoli di indagini approfondite. Background
Mutations to the complex telomere/telomerase cause the “Telomeropathy” that have different presentations but in all of them there is a telomere shortening.
Objective
To value the telomere’s length and the correlated clinical, hematological and genetic features, in a group of patients with clinical sign of dyskeratosis congenita (DC) or in patients with persistent cytopenia.
Patients and methods
the cytopenia was evaluated with the CTCAE 5.0 criteria and defined as immune if associated to auto immunity.
The phenotype’s anomalies were evaluated with a score defined as mild (<6 points), moderate (6-15) and severe (>15), depepending on the severity of the symptoms.
The telomeres were measured with the flow FISH.
The NGS panel evaluated 156 genetic variants associated with bone marrow failure, immunodeficiency/immune dysregulation.
Results
From April 2015 to February 2020 in the hematology center of the “Istituto G. Gaslini”, we recruited 55 patients, divided in 3 groups: 1° with normal telomere length (37/55); 2° with telomere length between the 1° and 5° percentile (3/55); 3° with telomere length < 1° percentile (15/55).
The alterations and the genes of DC where significantly more frequent in the groups 2 and 3 (p=0.04 and p=0.009), instead there weren’t differences in the cytopenia, the bone marrow failure and the phenotype score.
Lymphocytes B and NK were the most depleted classes in the groups 2 and 3 compared to the group 1. The immune cytopenia was most frequently associated to the immune dysregulation genes’ in the group 1 compared to the groups 2 and 3.
Good concordance for the combination of phenotype alterations and cytopenia; and for cytopenia + DC phenotype between the groups 2 and 3 (k=0.7).
Conclusions
Patients with even mild DC alterations need the measurement of the telomeres, while in the immune cytopenia the exam could not need to be performed. Even the telomere length < 5° percentile give the suspect of telomeropathies and needs othe
